摘要
目的探究2型糖尿病(T2DM)患者糖化血红蛋白(HbA_(1)c)水平与中性粒细胞/淋巴细胞比值(NLR)的相关性。方法选取2021年9月至2024年11月于三明市大田县总医院治疗的T2DM患者120例,按照HbA_(1)c水平分为控制良好组(HbA_(1)c≤7%)68例和控制不良组(HbA_(1)c>7%)52例,比较2组患者基础临床资料、NLR、生化指标并进行统计学分析。结果控制不良组的体质指数[(25.9±1.7)kg/m^(2)]、病程[(10.0±1.7)年]、甘油三酯[(2.36±0.27)mmol/L]、总胆固醇[(5.32±1.43)mmol/L]、中性粒细胞计数[(3.85±0.46)×10^(9)/L]、NLR(2.01±0.37)均高于控制良好组[(24.7±1.6)kg/m^(2)、(8.5±1.3)年、(2.08±0.36)mmol/L、(4.77±1.22)mmol/L、(3.23±0.43)×10^(9)/L、1.61±0.29](P<0.05);而受教育程度低于控制良好组(P<0.05);多因素logistic回归分析结果显示,病程较长[OR(95%CI)=1.765(1.351,2.301),P<0.001]、受教育程度为高中及以下[OR(95%CI)=0.619(0.393,0.924),P=0.038]、体质指数[OR(95%CI)=2.368(1.043,5.372),P=0.039]及NLR水平升高[OR(95%CI)=1.455(1.164,1.819),P=0.001]为T2DM患者血糖控制不良的独立危险因素(P<0.05);受试者工作特性曲线(ROC)显示,NLR预测T2DM患者血糖控制不良的曲线下面积(95%CI)为0.801(0.720,0.883)。结论T2DM患者HbA_(1)c水平与NLR密切相关,NLR可用于预测T2DM患者血糖控制效果。
Objective To investigate the correlation between glycosylated hemoglobin(HbA_(1)c)and neutrophil/lymphocyte ratio(NLR)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 120 patients with T2DM treated in General Hospital of Datian County from September 2021 to November 2024 were selected and divided into well-controlled group(HbA_(1)c≤7%)and poorly controlled group(HbA_(1)c>7%)according to HbA_(1)c level.Basic clinical data,NLR and biochemical indexes of the two groups were compared and analyzed statistically.Results Body mass index(BMI)of poorly controlled group(25.9±1.7)kg/m^(2)],duration[(10.0±1.7)years],triglycerides[(2.36±0.27)mmol/L],total cholesterol[(5.32±1.43)mmol/L],neutrophil count[(3.85±0.46)×10^(9)/L]、NLR(2.01±0.37)were higher than those in the control group[(24.7±1.6)kg/m^(2)、(8.5±1.3)years,(2.08±0.36)mmol/L、(4.77±1.22)mmol/L、(3.23±0.43)×10^(9)/L、1.61±0.29](P<0.05),while the education level was lower than that of the control group(P<0.05).Multivariate logistic regression analysis showed that longer disease duration[OR(95%CI)=1.765(1.351,2.301),P<0.001],lower education[OR(95%CI)=0.619(0.393,0.924),P=0.038],higher BMI[OR(95%CI)=2.368(1.043,5.372),P=0.039]and NLR[OR(95%CI)=1.455(1.164,1.819),P=0.001]were independent risk factors for poor glycemic control in T2DM patients(P<0.05).Receiver operating characteristic curve showed that the area under the curve of NLR(95%CI)predicting poor glycemic control in T2DM patients was 0.801(0.720,0.883).Conclusion HbA_(1)c level is closely related to NLR in patients with type 2 diabetes mellitus.NLR can be used to predict the effect of glycemic control in patients with type 2 diabetes mellitus.
