不同浓度红细胞对尿液干化学检测的影响分析

摘要 目的探讨不同浓度红细胞对FUS-1000全自动尿液分析系统干化学检测项目的影响。方法选取武汉市第一医院2024年11月至2025年3月尿常规检测阴性的41份样本为原始样本,加入不同浓度的红细胞(RBC)配制成人工血尿样本并依据所含RBC的浓度从高到底划分为组1、组2、组3和组4,测定各组人工血尿样本离心前后尿干化学检测结果,分析不同浓度RBC对尿干化学检测的影响及离心处理消除RBC干扰的效果。结果随着RBC浓度的增加,酮体(KET)、蛋白质(PRO)、白细胞酯酶(LEU)和葡萄糖(GLU)呈现不同程度的假阳性,KET在组1和组2的假阳性率均为100%、组3为95.12%、组4为2.44%;PRO在组1假阳性率为100%、组2和组3分别为80.49%、17.07%;LEU在组4中的假阳性率为4.88%,其他各组假阳性率均为100%;GLU在各组中的假阳性率依次为100%、95.12%、24.39%和0,且经ROC曲线分析,KET、PRO、LEU和GLU出现假阳性的RBC浓度阈值分别为12302.50个/μl、38677.50个/μl、6729.50个/μl和21968.50个/μl,尿胆原有少量呈现假阳性,亚硝酸盐、胆红素的结果不受RBC浓度影响。离心处理后PRO仅组1出现1例“trace”(极微量蛋白质)结果,其他项目各组假阳性率均为0,且离心处理前后假阳性率差异有统计学意义(P<0.05)。结论血尿可致部分干化学检测结果假阳性,在实际工作中遇到血尿尤其是超出RBC浓度阈值的血尿标本,需执行非离心和离心后两次检测,以便消除RBC对干化学测定的干扰。 Objective To explore the effect of varying concentrations of red blood cells(RBCs)on urine dry chemistry detection items using FUS-1000 automated urine analyzer.Methods A total of 41 samples with negative urine routine tests from November 2024 to March 2025 were selected as raw samples in the the Wuhan NO.1 Hospital.Artificial hematuria samples were prepared by spiking with different concentrations and were divided into Group 1,Group 2,Group 3,and Group 4 in descending order of RBC concentration.The effect of RBC for urine dry chemistry detection and the ability of centrifugation treatment on eliminating RBC interference were analyzed by detecting the urine dry chemistry results before and after centrifugation of artificial hematuria samples.Results With the increase of RBC concentration,ketone(KET),protein(PRO),white blood cell esterase(LEU)and glucose(GLU)showed varying degrees of false positives rate.The false positive rates of KET in Group 1 and Group 2 were both 100%,Group 3 was 95.12%,and Group 4 was 2.44%.The false positive rates of PRO in both Group 1 was 100%,Group 2 and 3 were 80.49%and 17.07%respectively;The false positive rate of LEU in Group 4 was 4.88%,while the false positive rates in all other groups were 100%.The false positive rates of GLU in each group were 100%,95.12%,24.39%,and 0,respectively.According to the ROC curve analysis,the RBC concentration thresholds of KET,PRO,LEU and GLU with false positives were 12302.50,38677.50,6729.50 and 21968.50 cells/μl,respectively.Urobilinogen(UBG)of a few samples showed false positives.The results of nitrite and bilirubin were not affected by RBC;After centrifugation,only Group 1 had 1"trace"result,while the false positive rate of other items was 0,and there was a statistically significant difference in the false positive rate before and after centrifugation treatment(P<0.05).Conclusion Hematuria can lead to false positive results in some urine dry chemistry items.During practical work,it is recommended to perform both non-centrifuged and centrifuged urinalysis for hematuria samples,particularly those exceeding established RBC thresholds,in order to minimize interference.
