摘要
乳房疾病在中医学中是“乳少”“乳溢”“乳癖”“乳岩”等发生于乳房部位的疾病的总称。作为临床常见疾病,近年来其发病率持续攀升,严重影响女性身体及心理健康。传统治疗方法如手术和药物虽有一定疗效,但其局限性和不良反应促使人们探索替代疗法。作为中医特色疗法,针灸在乳房疾病干预过程中优势显著,其中手太阳小肠经的少泽穴因与乳房生理病理紧密关联,成为临床高频选穴,疗效确切,但其现代医学作用机制尚需深入阐释。现代医学揭示,神经、内分泌、免疫三大系统通过神经递质、激素及细胞因子等介质相互调控,共同构成一个完整的调节系统网络,即神经–内分泌–免疫网络。刺激少泽穴所产生的信息效应通过神经–内分泌–免疫网络实现级联放大,多层次、多系统地输出到靶器官,改善其病理状态,从而治疗乳房疾病。文章结合中医对乳房疾病病因病机的认识,以神经–内分泌–免疫网络为研究框架,系统整合临床实践与实验研究成果,分别从神经、内分泌、免疫3个维度系统阐释针刺少泽穴治疗乳房疾病的机制,从整体观揭示少泽穴对乳房疾病的干预机制与生物学途径,以期为少泽穴治疗乳房疾病的研究提供可靠理论基础。
Breast diseases in traditional Chinese medicine refer to a general term for various diseases occurring in the breast area,including insufficient lactation,galactorrhea,breast lumps,and breast cancer.As common clinical diseases,their incidence rate is continuously rising,seriously affecting the physical and mental health of women.Although traditional treatment methods such as surgery and medication have certain therapeutic effects,their limitations and adverse reactions have prompted people to explore alternative therapies.As a characteristic therapy of traditional Chinese medicine,acupuncture has significant advantages in the intervention of breast diseases.Among them,the Shaoze acupoint(SI1)on the Hand Taiyang(太阳)Small Intestine Meridian,which is closely related to the physiology and pathology of the breast,has become a frequently selected point in clinical practice,with definite therapeutic effects.However,its modern medical mechanism of action still needs to be further explained.Modern medicine reveals that the nervous,endocrine,and immune systems are mutually regulated through neurotransmitters,hormones,and cytokines,etc.,forming a complete regulatory system network,namely the neuro-endocrine-immune network.The information effect generated by stimulating the Shaoze acupoint is amplified through the neuro-endocrine-immune network in a cascade manner,and is output to the target organ at multiple levels and systems,ameliorating its pathological state and thereby treating breast diseases.This article,based on the understanding of the etiology and pathogenesis of breast diseases in traditional Chinese medicine,takes the neuro-endocrine-immune network as the research framework,systematically integrates clinical practice and experimental research results,and systematically explains the mechanism of acupuncture at the Shaoze acupoint in treating breast diseases from the three dimensions of the nervous,endocrine,and immune systems.It reveals the intervention mechanism and biological pathways of the Shaoze acupoint on breast diseases from a holistic perspective,with the aim of providing reliable theoretical support for the research on the treatment of breast diseases by the Shaoze acupoint.
