摘要
刺血疗法为中医针刺疗法中的一种,是在人体特定的穴位及浅表脉络放出少量血液以达到泄热排毒之效,从而治疗疾病的一种方法。其适用病证十分广泛,临床操作简便易于实施,疗效迅速且显著,在治疗某些急慢病时可作为首选治法考虑。但有关临床施术部位的选取十分复杂,且面对同一种病况时常有多种选择,因此对于临床可施术部位的认知与探讨有着不可忽视的重要性。为了进一步了解刺血疗法的选穴原则,文章对刺血疗法的选穴原则进行了综述,总结了临床常用的相关选穴思路。文章从五输穴、循经选穴、阿是穴、病变局部、经验取穴、特殊部位几个角度进行分析述评,阐述了各种治疗优势方向及其适应病证,尤其是五输穴在刺血疗法选穴中的重要地位。文章亦通过对古籍中的描述及现代临床试验结论的结合探讨,分别论述了“井荥输经合”五类穴位不同的治疗功效,以供读者更好地理解与掌握刺血疗法选穴原则。鉴于在临床治疗过程中有不少注意事项需医生仔细甄别与谨记,文章对注意事项进行讨论以完备治疗过程的安全性与规范性,避免临床治疗过程中在选择刺血疗法时因甄别不明或操作不当而导致疗效不佳甚则引发治疗事故,为临床使用刺血疗法提供述评与参考。
Pricking blood therapy is a kind of traditional Chinese medicine acupuncture therapy.It is a method to treat diseases by releasing a small amount of blood at specific acupoints and superficial veins of the human body to achieve the effect of heat release and detoxification.It is applicable to a wide range of diseases,simple and easy to implement clinical operation,and has a rapid and significant curative effect.In the treatment of some acute and chronic diseases,it can be regarded as the preferred treatment.The selection of clinical acupoint location is very complex,and in the face of the same condition,there are often many choices,so it is necessary to explore the clinical acupoint location.In order to further understand the acupoint selection principle of blood pricking therapy,this article provides a comprehensive review of these principles and summarizes commonly used clinical approaches to acupoint selection.This article analyzes and evaluates several perspectives,including the Wushu acupoints(五输穴),meridian-based acupoint selection,Ashi acupoints,local lesions,empirical acupoint selection,and special areas.It elucidates the therapeutic advantages and indications of each perspective,with a particular focus on the significant role of the Wushu acupoints in the selection of acupoints of pricking blood therapy.This article also explores the different therapeutic effects of the five acupoints,namely“Jing(井),Ying(荥),Shu(输),Jing(经),He(合)”acupoints,through a combination of descriptions from ancient books and conclusions from modern clinical trials,to help readers better understand and grasp the principles of acupoint selection of pricking blood therapy.In view of many precautions that need to be carefully identified and remembered by doctors in the clinical treatment process,this article also discusses the precautions to improve the safety and standardization of the treatment process,and avoid the poor efficacy or even treatment accidents caused by unclear screening or improper operation of blood pricking therapy.
