摘要
目的研究吡非尼酮胶囊联合糖皮质激素对特发性肺间质纤维化(idiopathicpulmonary fibrosis,IPF)患者肺功能、肺部纤维化的影响。方法回顾性分析2021年5月—2024年2月在武平县医院接受治疗的85例特发性肺间质纤维化患者的临床资料,按照治疗方案的不同进行分组。对照组43例采用糖皮质激素治疗;观察组42例在糖皮质激素的基础上联合吡非尼酮胶囊治疗。比较两组临床疗效、肺功能、肺部纤维化及用药安全性。结果观察组临床总有效率为95.24%,高于对照组的79.07%,差异有统计学意义(P<0.05)。治疗后,观察组第1秒用力呼气量、最大呼气峰流速、用力肺活量、肺一氧化碳弥散量均高于对照组,差异有统计学意义(P<0.05);观察组Ⅲ型前胶原、层黏连蛋白、Ⅳ型胶原均低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论吡非尼酮胶囊联合糖皮质激素治疗效果显著,可有效抑制IPF患者肺部纤维化,促进肺功能好转,且用药安全性较好。
Objective To study the effect of pirfenidone capsule combined with glucocorticoid on pulmonary function and pulmonary fibrosis in patients with idiopathic pulmonary fibrosis(IPF).Methods The clinical data of 85 patients with idiopathic pulmonary interstitial fibrosis who were treated in Wuping County Hospital,Wuping from May 2021 to February 2024 were retrospectively analyzed.According to the treatment plan,43 patients in the control group were treated with glucocorticoid,and 42 patients in the observation group were treated with pirfenidone capsules on the basis of glucocorticoid.The clinical efficacy,pulmonary function,pulmonary fibrosis and drug safety were compared between the two groups.Results The total clinical efficacy rate in the observation group was 95.24%,which was higher than 79.07%in the control group(P<0.05).After treatment,the forced expiratory volume in 1 second,peak expiratory flow rate,forced vital capacity and diffusion capacity of the lung for carbon monoxidein the observation group were all higher than those in the control group(P<0.05).The levels of typeⅢprocollagen,laminin,and typeⅣcollagen in the observation group were all lower than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Pifenidone capsule combined with glucocorticoid has obvious therapeutic effect,which can effectively inhibit pulmonary fibrosis in IPF patients,promote the improvement of pulmonary function,and has good medication safety.
