摘要
清法作为中医治疗热证的核心治法,在骨伤科领域具有独特的理论价值与临床应用。清法经历代医家不断丰富其内涵,形成以“清消”与“清养”为核心的辨证体系。“清消”法针对热邪与兼邪胶结的病机,通过清热解毒、清热利湿、化瘀清热等策略,体现祛邪与调节并重的特点。“清养”法则注重热病后期的正虚邪恋,采用清营凉血和养阴清热等治法,兼顾清热与扶正。清法在针对骨伤病病程中出现热性症状的病理阶段有着较好的效果,展现了中医辨证论治的独特优势。
As the core therapeutic approach in traditional Chinese medicine(TCM)for treating heat-related syndromes,clearing method holds distinctive theoretical value and clinical applicability in the field of orthopedics and traumatology.Clearing method has been continuously enriched by physicians throughout history,forming a dialectical system centered on"clearing and elimination"and"clearing and nourishment"."Clearing and elimination"method targets pathological mechanism of heat evil combined with stagnation with superimposed evils,and this approach embodies the dual emphasis on eliminating pathogenic factor and regulatory balance by strategies such as clearing heat and detoxification,clearing heat and draining dampness,and dissipating blood stasis to clear heat."Clearing and nourishment"approach emphasizes syndrome of lingering pathogen due to deficient vital qi in the late stage of febrile diseases,and it adopts therapeutic methods such as clearing nutrient level and cooling blood,and nourishing yin and clearing heat,while simultaneously clearing heat and strengthening vital qi.Clearing method demonstrates favorable eficacy in pathological stages of bone injuries that involve heat manifestations,showcasing the unique advantage of TCM treatment based on syndrome differentiation.
