摘要
目的:探讨血脂、同型半胱氨酸(Hcy)及D-二聚体(D-D)联合检测在急性脑梗死诊断中的价值。方法:选取2020年3月至2022年2月在淮南新华医疗集团新华医院诊治的60例急性脑梗死患者及进行健康体检的40例健康人。将急性脑梗死患者作为脑梗死组,将健康人作为对照组。检测并对比两组血脂、Hcy及D-D水平,使用受试者工作特征(ROC)曲线分析各指标诊断急性脑梗死的效能。结果:脑梗死组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、Hcy、D-D水平高于对照组(P<0.05),而高密度脂蛋白胆固醇(HDL-C)水平低于对照组(P<0.05)。TC、TG、HDL-C、LDL-C、Hcy、D-D检测诊断急性脑梗死的曲线下面积(AUC)分别为0.820、0.705、0.854、0.855、0.882、0.899;联合预测因子诊断急性脑梗死的AUC最大,为0.935。结论:血脂、Hcy及D-D联合检测在急性脑梗死的诊断中具有较高的价值,可作为临床诊断急性脑梗死的重要辅助手段。
Objective:To explore the value of combined detection of blood lipids,homocysteine(Hcy),and D-dimer(D-D)in the diagnosis of acute cerebral infarction.Methods:A total of 60 patients with acute cerebral infarction and 40 healthy individuals undergoing health checkups at Xinhua Hospital of Huainan Xinhua Medical Group from March 2020 to February 2022 were selected.The acute cerebral infarction patients were assigned to the cerebral infarction group,and the healthy individuals to the control group.The levels of blood lipids,homocysteine,and D-dimer were measured and compared between the two groups.The efficacy of each indicator in diagnosing acute cerebral infarction was analyzed using receiver operating characteristic(ROC)curves.Results:The levels of total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),Hcy,and D-D in the cerebral infarction group were higher than those in the control group(P<0.05),while the level of high-density lipoprotein cholesterol(HDL-C)was lower than that in the control group(P<0.05).The areas under the curve(AUC)for diagnosing acute cerebral infarction were 0.820 for TC,0.705 for TG,0.854 for HDL-C,0.855 for LDL-C,0.882 for Hcy,and 0.899 for D-D.The combined predictor had the largest AUC for diagnosing acute cerebral infarction,at 0.935.Conclusion:Combined detection of blood lipids,Hcy,and D-D has high value in the diagnosis of acute cerebral infarction and can serve as an important auxiliary tool for clinical diagnosis of acute cerebral infarction.
