摘要
目的探讨加味定喘汤对呼吸道合胞病毒(respiratory syncytial virus,RSV)致小儿下呼吸道感染的临床疗效。方法选取呼吸道合胞病毒下呼吸道感染患儿92例,采用随机数表法分为对照组与中药组,各46例。对照组给予常规治疗,中药组在对照组基础上联合加味定喘汤治疗。比较2组临床疗效结果,治疗前后临床症状改善时间、炎症指标、气道阻力指标及不良反应发生率。结果中药组临床疗效结果总有效率(97.83%,45/46)高于对照组(84.78%,39/46)(P<0.05)。中药组咳嗽消失时间、喘息缓解时间、肺部体征消失时间均低于对照组(P<0.05)。治疗后中药组总气道阻力(R5)、外周弹性阻力(X5)、呼吸总阻抗(Z5)均低于对照组(P<0.05)。治疗后中药组TNF-α、IL-6水平均低于对照组(P<0.05)。中药组的不良事件发生率与对照组比较,差异无统计学意义(P>0.05)。结论加味定喘汤可显著缩短呼吸道合胞病毒感染患儿临床症状持续时间,降低炎症反应,促进气道功能恢复,且安全性良好,值得临床推广。
Objective To investigate the clinical efficacy of modified Dingchuan Decoction in the treatment of lower respiratory tract infections(LRTIs)in children caused by respiratory syncytial virus(RSV).Methods A total of 92 pediatric patients with RSV-induced LRTIs admitted between January 2024 and April 2025 were enrolled and randomly divided into a control group(n=46,conventional therapy)and an herbal medicine group(n=46,conventional therapy plus modified Dingchuan Decoction).The clinical efficacy between the two groups,the symptom resolution time,inflammatory markers,airway resistance parameters,and adverse event rates were compared before and after treatment.Results The total effective rate(97.83%,45/46 cases)was significantly higher in the herbal medicine group than that in the control group(84.78%,39/46 cases)(P<0.05).The herbal medicine group exhibited shorter cough disappearance time,wheezing relief time,and lung sign clearance time compared to the control group(P<0.05).Posttreatment R5,X5,and Z5 values were significantly lower in the herbal medicine group(P<0.05),along with reduced TNF-αand IL-6 levels(P<0.05).No statistically significant difference was observed in adverse event incidence between the two groups(P>0.05).Conclusion Modified Dingchuan Decoction significantly accelerates the symptom resolution,mitigates inflammatory responses,improves airway function,and exhibits favorable safety in RSV-infected children,supporting its clinical application.
