摘要
目的观察三维超声对女性盆底肌功能障碍性疾病(FPFD)的诊断效果。方法选择2023年1月至2024年1月河南省直第三人民医院收治的88例疑似FPFD就诊者为研究对象,最终临床确诊82例为FPFD患者,其中盆腔脏器脱垂(POP)组56例,压力性尿失禁(SUI)组26例,另选79例盆底功能正常者为对照组,统计三维超声的诊断效能,采集三组Valsalva与静息下肛提肌裂孔面积(S),测量膀胱颈移动度(BND),记录子宫前位与后位情况。结果以综合诊断为金标准,三维超声诊断POP、SUI的效能较高。Valsalva及静息下,POP组、SUI组肛提肌裂孔面积(S)高于对照组,BND高于对照组,差异有统计学意义(P<0.05)。POP组、SUI组子宫前位比例低于对照组,差异有统计学意义(P<0.05)。结论经会阴三维超声通过量化评估肛提肌裂孔面积和膀胱颈移动度等,可有效区分FPFD与健康人群,其诊断效能与临床综合诊断标准具有较高的一致性。
Objective To observe the diagnostic effect of three-dimensional ultrasound on female pelvic floor muscle dysfunction(FPFD).Methods A total of 88 suspected patients with FPFD admitted to the Third People’s Hospital of Henan Province from January 2023 to January 2024 were selected as the research subjects.Eventually,82 cases were clinically diagnosed as FPFD patients,including 56 cases in the pelvic organ prolapse(POP)group and 26 cases in the stress urinary incontinence(SUI)group.Another 79 patients with normal pelvic floor function were selected as the control group.The diagnostic efficacy of three-dimensional ultrasound was analyzed.The Valsalva and the area(S)of the levator ANI muscle hiatus at rest in three groups were collected.The bladder neck mobility(BND)was measured,and the anterior and posterior positions of the uterus were recorded.Results Taking comprehensive diagnosis as the gold standard,the efficacy of three-dimensional ultrasound in diagnosing POP and SUI was relatively high.In Valsalva and at rest,the area(S)of the levator ANI muscle hiatus in the POP group and the SUI group was higher than that in the control group,and the BND was also higher than that in the control group.The differences were significant(P<0.05).The proportion of anterior uterus in the POP group and the SUI group was lower than that in the control group,and the differences were significant(P<0.05).Conclusions Transperineal three-dimensional ultrasound can effectively distinguish FPFD from healthy people by quantitatively evaluating the area of the levator ANI muscle hiatus and the mobility of the bladder neck,etc.Its diagnostic efficacy has a high consistency with the comprehensive clinical diagnostic criteria.
