摘要
探讨慢性心力衰竭(CHF)患者及其配偶的二元应对与心理弹性的关系。采用便利抽样法,对122对CHF夫妻施测二元应对量表与心理弹性量表,基于主客体互倚模型分析双方交互影响。结果显示,夫妻双方在二元应对总分和心理弹性总分上差异无统计学意义(P>0.05),但在压力沟通(t=2.38,P=0.01)和共同应对(t=-3.07,P=0.01)维度上存在显著差异。APIM分析显示,双方二元应对不仅正向预测自身心理弹性(β=0.594、0.603,P<0.001),也显著影响对方心理弹性(β=0.292、0.557,P<0.05)。进一步Bootstrap检验提示,患者为主-客体对偶模式,配偶为混合模式。研究发现,CHF患者及其配偶二元应对能力中等,心理弹性偏低,双方二元应对与彼此心理弹性存在双向正向交互效应,医疗工作者应将夫妻视为整体制定协同干预方案。
To explore the relationship between dyadic coping and psychological resilience in patients with chronic heart failure(CHF)and their spouses.A convenience sampling method was used to recruit 122 CHF patient-spouse dyads,who were assessed using the Dyadic Coping Scale and Resilience Scale.The Actor-Partner Interdependence Model(APIM)was applied to analyze the mutual interaction between the two parties.The results showed that there were no statistically significant differences between the patient and spouse in the total scores of dyadic coping and psychological resilience(P>0.05),but significant differences were observed in the dimensions of stress communication(t=2.38,P=0.01)and common coping(t=-3.07,P=0.01).APIM analysis revealed that dyadic coping of each party not only positively predicted their own psychological resilience(β=0.594,0.603;P<0.001)but also significantly influenced the other's psychological resilience(β=0.292,0.557;P<0.05).Further Bootstrap testing indicated that the patient-spouse dyad followed an actor-partner model for patients and a mixed model for spouses.The study concluded that CHF patients and their spouses have moderate dyadic coping ability and low psychological resilience,and there is a bidirectional positive interaction effect between dyadic coping and each other's psychological resilience.Healthcare providers should consider the patient-spouse dyad as a whole when formulating collaborative intervention programs.
