摘要
目的 了解某国企综合三级医院的医务人员手卫生执行现状,以便为日后提升手卫生依从性提供数据支持和参考依据。方法 于2024年1—7月通过采用现场观察单盲试验的方法对某国企综合三级医院268名在岗医务人员的手卫生执行现状进行调查与分析。结果 共计观察458次手卫生应执行次数,实际执行次数为206次,总体手卫生依从率为44.98%,不同科室、性别、职业医务人员手卫生依从率比较,差异有统计学意义(P<0.001)。无菌操作前和接触患者体液后的手卫生依从率较高,占比为80.00%、100%,不同手卫生时刻下医务人员手卫生执行情况比较,差异有统计学意义(P<0.001)。共随机抽查了183名医务人员的洗手情况,其中173名的操作达到了合格标准,整体合格率为94.54%。手卫生知识竞赛中,正确率达93.66%。对71名医务人员的手进行三磷酸腺苷(adenosine triphosphate,ATP)荧光检测,初次检测合格率为49.30%。结论 某国企综合三级医院医务人员手卫生依从性总体上有待提高,且ATP初次检测合格率欠佳,可根据具体的执行状况提高手卫生质量,进而改善手卫生依从性。
Objective To understand the current situation of hand hygiene implementation among medical staff in a state-owned enterprise general tertiary hospital,so as to provide data support and reference basis for improving hand hygiene compliance in the future.Methods From January to July 2024,the current status of hand hygiene implementation among 268 on-the-job medical staff in a state-owned enterprise general tertiary hospital was investigated and analyzed through on-site observation and single-blind trials.Results A total of 458 opportunities for hand hygiene were observed,with 206 actual executions,resulting in an overall hand hygiene compliance rate of 44.98%.Statistically significant differences were found in hand hygiene compliance rates among medical staff from different departments,genders,and occupations(P<0.001).The compliance rates were relatively high before aseptic procedures and after contact with patient body fluids,accounting for 80.00%and 100%respectively.Moreover,statistically significant differences were observed in the implementation of hand hygiene by medical staff at different hand hygiene moments(P<0.001).A total of 183 medical staff were randomly selected to check their handwashing practices,among whom 173 met the qualified standards,with an overall pass rate of 94.54%.In the hand hygiene knowledge contest,the correct rate reached 93.66%.Additionally,a total of 71 medical staff underwent adenosine triphosphate(ATP)fluorescence detection for their hands,and the initial pass rate was 49.30%.Conclusion The hand hygiene compliance of medical staff in a state-owned enterprise general tertiary hospital generally needs to be improved,and the pass rate of the initial ATP test is unsatisfactory.The quality of hand hygiene can be improved according to the specific implementation status,thereby enhancing hand hygiene compliance.
