摘要
目的:探究CT动态增强扫描在肝硬化(HC)合并原发性肝癌(PLC)患者中的诊断效能。方法:选取本院2021年1月~2024年1月接受检查的80例疑似HC合并PLC患者作为研究对象,所有患者均行CT动态增强扫描。以病理检查结果为金标准,观察CT动态增强扫描诊断效能,对比肝脏血流灌注参数,并分析CT动态增强扫描血流灌注参数对诊断HC合并PLC患者的预测价值。结果:CT动态增强扫描检出PLC阳性45例,阴性35例,检测灵敏度78.85%、特异度85.71%、准确度81.25%、漏诊率21.15%及误诊率14.29%;HC组患者HAP、HPI值均低于HC合并PLC组(P<0.05),TLP、PVP值均高于HC合并PLC组(P<0.05);ROC曲线结果显示,联合检测的AUC为0.837,其灵敏度最高,特异度为0.714(P<0.05)。结论:CT动态增强扫描对HC合并PLC患者具有较好的诊断价值,肝脏血流灌注参数的特征性表现对于HC合并PLC诊断和肝功能分级提供了重要参考信息。
Objective:To investigate the diagnostic efficacy of dynamic contrast-enhanced CT in patients with liver cirrhosis(HC)complicated by primary liver cancer(PLC).Methods:80 patients suspected of having HC with PLC who underwent examination at our hospital from January 2021 to January 2024 were enrolled.All patients underwent dynamic contrast-enhanced CT.Using pathological examination results as the gold standard,we evaluated the diagnostic efficacy of dynamic contrast-enhanced CT,compared hepatic perfusion parameters,and analyzed the predictive value of perfusion parameters from dynamic contrastenhanced CT for diagnosing HC with PLC.Results:CT dynamic contrast-enhanced scanning detected 45 positive cases and 35 negative cases of PLC,with a detection sensitivity of 78.85%,specificity of 85.71%,accuracy of 81.25%,missed diagnosis rate of 21.15%,and misdiagnosis rate of 14.29%;The HAP and HPI values of patients in the HC group were lower than those in the HC combined with PLC group(P<0.05),while the TLP and PVP values were higher than those in the HC combined with PLC group(P<0.05);The ROC curve results showed that the AUC of the combined detection was 0.837,with the highest sensitivity and specificity of 0.714(P<0.05).Conclusion:Dynamic contrast-enhanced CT demonstrates good diagnostic value for patients with HC complicated by PLC.Characteristic liver perfusion parameters provide important reference information for diagnosing PLC in HC patients and for liver function staging.
