摘要
目的:对比磁共振成像(MRI)与计算机体层成像(CT)检查在原发性肝癌诊断中的价值。方法:选取2022年1月~2024年3月在本院接受检查的疑似原发性肝癌患者189例。比较两种诊断效能并分析影像学图像特征。结果:189例疑似原发性肝癌患者经金标准诊断阳性155例,阴性34例。MRI诊断真阳性152例,真阴性29例;CT诊断真阳性153例,真阴性26例;两组灵敏度、特异度、准确度比较,差异无统计学意义(P>0.05);MRI对肝内病灶、肝外病灶检出数量均多于CT检查(P<0.05);189例疑似原发性肝癌患者中,肿瘤病灶直径≤2cm有82例,病灶>2cm有107例。肿瘤病灶≤2cm时,MRI检出率高于CT检查(P<0.05),肿瘤病灶>2cm,两者检出率差异无统计学意义(P>0.05)。结论:原发性肝癌使用MRI的诊断价值高于CT检查。
Objective:To compare the value of magnetic resonance imaging(MRI)and computed tomography(CT)in the diagnosis of primary liver cancer.Methods:A total of 189 patients suspected of having primary liver cancer who underwent examinations in our hospital from January 2022 to March 2024 were selected.The diagnostic efficacy of the two methods was compared,and the imaging image characteristics were analyzed.Results:Among the 189 patients suspected of having primary liver cancer,155 cases were diagnosed as positive by the gold standard,and 34 cases were diagnosed as negative.The true positive rate of MRI was 152 cases,and the true negative rate was 29 cases;The true positive rate of CT was 153 cases,and the true negative rate was 26 cases;There was no statistically significant difference in sensitivity,specificity,and accuracy between the two groups(P>0.05);MRI detected more liver lesions within the liver and outside the liver than CT(P<0.05);Among the 189 patients suspected of having primary liver cancer,82 had tumor lesions≤2cm in diameter,and 107 cases had tumor lesions>2cm.When the tumor lesion diameter was≤2cm,the detection rate of MRI was higher than that of CT(P<0.05),and there was no statistically significant difference in the detection rate between the two methods when the tumor lesion diameter was>2cm(P>0.05).Conclusion:The diagnostic value of MRI is higher than that of CT in primary liver cancer.
