摘要
目的研究养阴通络汤联合西药治疗脑梗死稳定期合并2型糖尿病(type 2 diabetes mellitus,T2DM)的疗效及对患者血液流变学、血糖和血脂的影响。方法选取2023年1月—2024年6月安溪县中医院收治的84例脑梗死稳定期合并T2DM患者为研究对象。根据不同治疗方法分为观察组与对照组,各42例。对照组实施丹参注射液治疗,观察组实施丹参注射液+养阴通络汤治疗。对比两组临床症状、糖脂代谢情况、血液流变学指标。结果治疗后,两组偏瘫、失语、意识混乱、美国国立卫生研究院卒中量表评分均降低,且观察组低于对照组,差异均有统计学意义(P均<0.05)。治疗后,两组糖化血红蛋白、空腹血糖、总胆固醇、甘油三酯、低密度脂蛋白胆固醇均降低,且观察组低于对照组,差异均有统计学意义(P均<0.05)。治疗后,两组红细胞压积、高切全血黏度、低切全血黏度、血浆黏度均降低,且观察组低于对照组,差异均有统计学意义(P均<0.05)。结论对脑梗死稳定期合并T2DM患者采取丹参注射液联合养阴通络汤治疗,能有效减轻脑梗死症状,促进神经功能恢复,并调节血脂代谢,改善血液流变学指标。
Objective To study the curative effect of Yangyin Tongluo Decoction combined with western medicine in the treatment of stable cerebral infarction with type 2 diabetes mellitus(T2DM)and its effect on hemorheology,blood glucose and blood lipid.Methods A total of 84 patients with stable cerebral infarction complicated with T2DM admitted to Anxi County Hospital of Traditional Chinese Medicine from January 2023 to June 2024 were selected as the study subjects.According to different treatment methods,they were divided into observation group and control group,with 42 cases in each group.The control group was treated with Danshen injection,and the observation group was treated with Danshen injection+Yangyin Tongluo Decoction.The clinical symptoms,glucose and lipid metabolism and hemorheology indexes of the two groups were compared.Results After treatment,the scores of hemiplegia,aphasia,confusion and National Institutes of Health Stroke Scale in the two groups decreased,and those in the observation group were lower than the control group,the differences were statistically significant(all P<0.05).After treatment,the levels of glycated hemoglobin A1c,fasting plasma glucose,total cholesterol,triglyceride and low-density lipoprotein cholesterol in the two groups decreased,and those in the observation group were lower than the control group,the differences were statistically significant(all P<0.05).After treatment,the hematocrit,high shear whole blood viscosity,low shear whole blood viscosity and plasma viscosity of the two groups decreased,and those in the observation group were lower than the control group,the differences were statistically significant(all P<0.05).Conclusion Danshen injection combined with Yangyin Tongluo Decoction can effectively reduce the symptoms of cerebral infarction,promote the recovery of neurological function,regulate blood lipid metabolism and improve hemorheological indexes in patients with stable cerebral infarction complicated with T2DM.
