摘要
张伟教授基于“瘀毒”辨治肺结节,提出其核心病机为肺气虚损、痰瘀毒邪痹阻肺络,属本虚标实之证。临床以“补肺为宗,逐瘀攻毒”为法,强调补肺健脾以固本元、调肝益肾以通气血、调脾攻毒以散结消癥。结合经典医理与临床实践创制“化结散膏”,逐病之瘀毒、补肺之元气,共奏攻补兼施之效,从而“扶正不留邪,祛邪不伤正”。临证注重四诊合参,针对痰瘀毒邪兼夹证候灵活加减,巧用虫类之药搜剔伏邪,拔毒外出。为治疗肺结节之久瘀顽疾提供了新思路,附验案1则。
Based on the theory of“blood stasis and toxin”,Professor ZHANG Wei has developed a differentiation and treatment approach for pulmonary nodule.He proposes that the core pathogenesis lies in lung qi deficiency with stagnation and obstruction of lung collaterals by phlegm,blood stasis and toxin pathogens,which is characterized by the syndrome of fundamental deficiency and symptomatic excess.Clinically,he employs the therapeutic principle of“tonifying the lungs as the fundamental principle while eliminating stasis and attacking toxins”,emphasizing the importance of reinforcing lung and spleen to consolidate constitutional foundation;regulating liver and kidney to promote qi and blood circulation;harmonizing spleen function and eliminating toxins to resolve nodules.Combining classic medical principles with clinical practice,he develops“Huajie Powder Ointment(化结散膏)”,which eliminates the stasis and toxins of the disease and replenishes the vital energy of the lungs,achieving a combined effect of attacking and tonifying,thereby“supporting the body’s healthy qi without leaving any pathogenic factors and eliminating pathogenic factors without harming the body’s healthy qi.”In clinical practice,Professor ZHANG emphasizes the integration of the four diagnostic methods,and flexibly adjusts the prescription according to the coexisting symptoms of phlegm,blood stasis and toxins,and applies insect-derived medications to eliminate latent pathogens strategically,which provides novel therapeutic strategies for the treatment of chronic and stubborn refractory pulmonary nodule.One test case is attached.
