鼻咽癌组织中P物质和神经激肽-1受体表达与鼻咽癌患者放化疗预后的相关性分析

摘要 目的分析鼻咽癌(nasopharyngeal carcinoma,NPC)组织中P物质(substance P,SP)和神经激肽-1受体(neurokinin-1 receptor,NK-1R)表达与NPC患者放化疗预后的相关性。方法选取2018年1月至2022年9月新余市人民医院收治的100例NPC放化疗患者作为研究对象,放化疗治疗结束后随访1年,分析患者预后情况,采用单因素、多因素二元Logistic回归分析NPC患者放化疗预后的危险因素,构建预后预测模型并绘制受试者工作特征(receiver operating characteristic,ROC)曲线分析SP、NK-1R蛋白表达阳性及预测模型对NPC放化疗患者预后的预测价值。结果随访1年,100例NPC患者中,预后良好86例;预后不良14例,其中复发11例,发生转移3例。单因素分析结果显示,T分期、M分期和NPC组织SP、NK-1R miRNA表达量及SP、NK-1R蛋白表达阳性均为NPC患者放化疗预后的影响因素(P<0.05)。多因素二元Logistic回归分析显示,SP、NK-1R蛋白表达阳性均为NPC患者放化疗预后的独立危险因素(P<0.05)。ROC曲线分析结果显示,SP、NK-1R蛋白表达阳性预测NPC患者放化疗预后的曲线下面积分别为0.761、0.713,预测模型验证结果为0.842,预测价值较高。结论SP、NK-1R蛋白阳性表达与NPC患者放化疗预后相关,二者对患者NPC复发或转移具有预警作用。 Objective To analyze the correlation between the expressions of substance P(SP)and neurokinin-1 receptor(NK-1R)in nasopharyngeal carcinoma(NPC)tissues and the prognosis of radiotherapy and chemotherapy in NPC patients.Methods A total of 100 patients with NPC undergoing radiotherapy and chemotherapy admitted to Xinyu People's Hospital from January 2018 to September 2022 were selected as the research subjects,the prognosis of the patients was analyzed by following up for one year after the end of radiotherapy and chemotherapy,univariate and multivariate binary Logistic regression were used to analyze the risk factors for the prognosis of NPC patients undergoing radiotherapy and chemotherapy,the prognostic prediction model was constructed and the receiver operating characteristic(ROC)curve was drawn to analyze the positive expression of SP and NK-1R proteins and the predictive value of the prediction model for the prognosis of NPC patients undergoing radiotherapy and chemotherapy.Results After one year follow-up,among 100 cases of NPC patients,86 had a good prognosis;14 had a poor prognosis,including 11 patients with recurrence and 3 patients with metastasis.Univariate analysis showed that the T stage and M stage,the expression levels of SP and NK-1R miRNA in NPC tissues,and the positive protein expressions of SP and NK-1R were all factors affecting the prognosis of radiotherapy and chemotherapy in NPC patients(P<0.05).Multivariate binary Logistic regression analysis showed that the positive expressions of SP and NK-1R proteins were independent risk factors affecting the prognosis of radiotherapy and chemotherapy in NPC patients(P<0.05).The results of ROC curve analysis showed that the area under the curve of positive expressions of SP and NK-1R proteins for predicting the prognosis of radiotherapy and chemotherapy in NPC patients were 0.761 and 0.713,respectively,and the verification result of the prediction model was 0.842,with a relatively high predictive value.Conclusion The positive expressions of SP and NK-1R proteins are related to the prognosis of radiotherapy and chemotherapy in NPC patients,and the positive expressions of both have an early warning effect on the recurrence or metastasis of NPC in patients.
