摘要
目的:评价并总结肝硬化失代偿期患者自我管理的相关证据。方法:按照“6S”证据模型系统检索UpToDate临床决策网站、国内外指南网站、专业协会网站以及Cochrane Library、PubMed、中国生物医学文献数据库、中国知网及万方数据知识服务平台有关肝硬化自我管理相关文献,文献类型包括临床决策、系统评价、指南、专家共识及原始研究等,检索时限为建库至2024年2月1日。由2名研究者独立进行文献质量评价和资料提取。结果:共纳入19篇文献,其中临床决策3篇、指南10篇、专家共识2篇、系统评价1篇、原始研究3篇。从接受多学科管理、学习肝硬化失代偿期相关知识、用药管理、症状监测、营养与饮食、生活方式、筛查与随访以及移植准备8个方面汇总了35条最佳证据。结论:本研究总结了肝硬化失代偿期患者自我管理的最佳证据,为临床医护人员规范患者自我管理指导、动态评估患者自我管理状况提供了依据。
Objective:To evaluate and synthesize the available evidence on self-management among patients with decompensated liver cirrhosis.Methods:According to the"6S"evidence model system,relevant literature on self-management of liver cirrhosis was searched on UpToDate clinical decision websites,guideline websites,professional association websites,as well as Cochrane Library,PubMed,China Biomedical Literature Database,China National Knowledge Infrastructure,and Wanfang Data Knowledge Service Platform.The types of literature included clinical decision-making,systematic reviews,guidelines,expert consensus,and original researches.The search time limit was from the establishment of the database to February 1,2024.Two researchers independently conducted literature quality evaluation and data extraction.Results:A total of 19 articles were included,including 3 clinical decision-making articles,10 guidelines,2 expert consensus articles,1 systematic review article,and 3 original study article.35 pieces of best evidence were summarized from eight aspects:receiving multidisciplinary management,learning about decompensated liver cirrhosis,medication management,symptom monitoring,nutrition and diet,lifestyle,screening and follow-up,and transplant preparation.Conclusion:This study summarizes the best evidence for self-management in patients with decompensated cirrhosis,which can provide a basis for clinical medical staff to standardize patient self-management guidance and dynamically evaluate patient self-management status.
