虚拟现实训练对脑卒中患者平衡能力影响的系统综述与Meta分析

摘要 目的:评价虚拟现实训练对脑卒中患者平衡能力恢复的疗效。方法:通过检索PubMed、Web of science、Med‐line、CBM、中国知网、万方以及维普等中英文数据库检索关于虚拟现实训练改善脑卒中患者平衡与步态稳定性的公开发表的随机对照试验(RCT),年限设置为建库至2024年8月。由2名研究者根据纳排标准逐步独立筛选论文,并提取数据,采用Revman5.4软件继续Meta分析。结果:共纳入11项RCT研究,样本量共502例,试验组247例,对照组255例。,Meta分析结果如下:虚拟现实训练干预组的Berg平衡量表得分(SMD=1.41,95%CI[0.80,2.02],Z=4.55,P<0.01)与起立-行走试验(SMD=-0.69,95%CI[-1.19,-0.20],Z=3.23,P<0.01)均高于对照组。亚组分析显示低频次时间适中的训练能够有效提高患者Berg平衡量表得分,而高频次时间适中的训练能够有效提高患者TUG成绩。结论:虚拟现实技术在改善脑卒中患者的步态稳定性和平衡能力方面表现出显著优势,且在一定程度上优于传统康复训练。然而,关于不同干预时长与强度组合对康复效果的影响,尚缺乏系统性的研究,仍需进一步探索以明确其最优干预方案。 Objective:To evaluate the efficacy of virtual reality training on the recovery of balance ability in stroke patients.Methods:Randomized controlled trials(RCTs)published in PubMed,Web of Science,Medline,CBM,CNKI,Wanfang,and VIP databases,from the establishment of the databases to August 2024,on the improvement of balance and gait stability in stroke patients through virtual reality training were retrieved.Two researchers independently screened the papers according to the inclusion and exclusion criteria and extracted the data.Meta-analysis was conducted using Revman 5.4 software.Results:A total of 11 RCTs were included,comprising a sample size of 502 cases,with 247 cases in the intervention group and 255 cases in the control group.The results of the meta-analysis are as follows:The Berg Balance Scale score(SMD=1.41,95%CI[0.80,2.02],Z=4.55,P<0.01)and the Timed Up and Go Test(SMD=-0.69,95%CI[-1.19,-0.20],Z=3.23,P<0.01)of the virtual reality training intervention group were higher than those of the control group.Subgroup analysis revealed that low-frequency and moderate-duration training were effective in improving the Berg Balance Scale score of patients,while high-frequency and moderate-duration training could effective in improving the Timed Up and Go Test score of patients.Conclusion:Virtual reality technology demonstrates significant advantages in enhancing the gait stability and balance abilities of stroke patients,and is superior to traditional rehabilitation training to a certain extent.However,there is a lack of systematic research on the impact of different intervention durations and intensities on rehabilitation outcomes,and further exploration is needed to determine the optimal intervention plan.
