上皮性卵巢癌患者血清CCL20、DKK1水平与临床病理特征及术后复发的关系

The relationship between serum C-C motif chemokine ligand 20 and recombinant human Dickkopf-related protein 1 expression levels with clinical pathological features and postoperative recurrence in patients with epithelial ovarian cancer

摘要 目的 探究上皮性卵巢癌(EOC)患者血清C-C基序趋化因子配体20(CCL20)、重组人Dickkopf相关蛋白1(DKK1)的表达水平与临床病理特征及术后复发的相关性。方法 选取2017年5月至2020年4月邯郸市第一医院收治的110例EOC患者作为研究对象。对EOC患者术后随访3年,记录术后复发情况。采用酶联免疫吸附试验(ELISA)法检测血清CCL20、DKK1表达水平;比较未复发组和复发组临床病理特征及血清CCL20、DKK1表达水平,分析血清CCL20、DKK1与临床病理特征的相关性;采用受试者工作特征(ROC)曲线分析血清CCL20、DKK1表达水平对EOC患者术后复发的预测价值,多因素Cox回归分析影响EOC患者术后复发的因素。结果 与未复发组比较,EOC患者术后复发组血清CCL20、DKK1表达水平均显著升高(P<0.05);两组国际妇产科联盟(FIGO)分期、淋巴结转移、肿瘤分化程度比较差异显著(P<0.05);肿瘤分化低、FIGO分期高、淋巴结转移的EOC患者CCL20、DKK1表达水平升高(P<0.05);血清CCL20、DKK1表达水平与FIGO分期、淋巴结转移呈正相关,与肿瘤分化程度呈负相关(P<0.05);血清CCL20、DKK1单独及二者联合预测EOC患者术后复发的曲线下面积(AUC)分别为0.939、0.973、0.998;血清CCL20、DKK1及FIGO分期均是EOC患者术后复发的危险因素(P<0.05)。结论 EOC术后复发患者血清CCL20、DKK1表达水平升高,二者表达水平与EOC患者肿瘤分化程度、淋巴结转移、FIGO分期及术后复发有关,可辅助预测EOC患者术后复发。 Objective To investigate the correlation between the expression levels of serum C-C motif chemokine ligand 20(CCL20)and recombinant human Dickkopf-related protein 1(DKK1)with clinical pathological features and postoperative recurrence in patients with epithelial ovarian cancer(EOC).Methods A total of 110 EOC patients admitted to Handan First Hospital from May 2017 to April 2020 were included as the study subjects.EOC patients were followed up for 3 years after surgery,and postoperative recurrence was recorded.Enzyme-linked immunosorbent assay(ELISA)was applied to detect the expression levels of serum CCL20 and DKK1.The clinical pathological features,and the expression levels of serum CCL20 and DKK1 were compared between the non-recurrence group and the recurrence group.The correlation between serum CCL20,DKK1 with clinical pathological features was analyzed.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum CCL20 and DKK1 expression levels for postoperative recurrence in EOC patients.Multivariate Cox regression was applied to analyze the factors affecting postoperative recurrence in EOC patients.Results Compared with the non-recurrence group,the expression levels of serum CCL20 and DKK1 of EOC patients in the postoperative recurrence group were significantly increased(P<0.05).There were significant differences in international federation of gynecology and obstetrics(FIGO)staging,lymph node metastasis,and tumor differentiation degree between the two groups(P<0.05).The expression levels of CCL20 and DKK1 were increased in EOC patients with low tumor differentiation,high FIGO staging,and lymph node metastasis(P<0.05).The expression levels of serum CCL20 and DKK1 were positively correlated with FIGO staging and lymph node metastasis,and negatively correlated with tumor differentiation degree(P<0.05).The area under the curve(AUC)of serum CCL20,DKK1 alone,and their combination in predicting postoperative recurrence in EOC patients was 0.939,0.973,and 0.998,respectively.Serum CCL20,DKK1,and FIGO staging were all risk factors for postoperative recurrence in EOC patients(P<0.05).Conclusions The expression levels of serum CCL20 and DKK1 in patients with postoperative recurrence of EOC are increased,and their expression levels are related to tumor differentiation,lymph node metastasis,FIGO staging,and postoperative recurrence of EOC patients,which can assist to predict postoperative recurrence in EOC patients.
