摘要
目的分析布地奈德福莫特罗吸入粉雾剂联合噻托溴铵吸入粉雾剂治疗稳定期慢阻肺合并糖尿病的临床效果。方法按照随机数字表法将2020年1月-2024年3月上饶市广丰区人民医院收治的66例稳定期慢阻肺合并糖尿病患者分为对照组和研究组,每组33例。两组均给予二甲双胍治疗,在此基础上,对照组应用布地奈德福莫特罗吸入粉雾剂治疗,研究组应用布地奈德福莫特罗吸入粉雾剂联合噻托溴铵吸入粉雾剂治疗。比较两组临床疗效、肺功能[用力肺活量(FVC)、第1秒钟用力呼气量(FEV_(1))、1秒率(FEV_(1)/FVC)、肺活量(VC)]、血糖水平[空腹血糖(FBG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)]、并发症(腹泻、恶心、低血糖)发生率。结果与对照组治疗总有效率的72.73%比较,研究组治疗总有效率更高,占比为96.97%(P<0.05);与对照组治疗后肺功能指标及血糖水平比较,研究组FVC、FEV_(1)、FEV_(1)/FVC、VC更高,FBG、2 hPG、HbA1c更低(P<0.05);与对照组并发症发生率的30.30%比较,研究组并发症发生率更低,占比为6.06%(P<0.05)。结论布地奈德福莫特罗吸入粉雾剂联合噻托溴铵吸入粉雾剂治疗稳定期慢阻肺合并糖尿病的临床效果确切,能够提高患者肺功能,降低患者血糖水平,且安全性较高。
Objective To analyze the clinical effect of budesonide and formoterol fumarate powder for inhalation combined with tiotropium bromide powder for inhalation in the treatment of stable chronic obstructive pulmonary disease complicated with diabetes mellitus.Methods From January 2020 to March 2024,66 patients with stable chronic obstructive pulmonary disease and diabetes mellitus admitted to Shangrao Guangfeng District People′s Hospital were selected as the research objects,and they were divided into control group and study group by using the random number table method,33 patients per group.Both groups were treated with metformin,on this basis,patients in the control group were treated with budesonide and formoterol fumarate powder for inhalation,and patients in the study group were treated with budesonide and formoterol fumarate powder for inhalation combined with tiotropium bromide powder for inhalation.The clinical efficacy,pulmonary function[forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),forced expiratory volume in one second(FEV_(1)/FVC),vital capacity(VC)],blood glucose level[fasting blood glucose(FBG),2 h postprandial blood glucose(2 hPG),glycosylated hemoglobin(HbA1c)]and incidence of complications(diarrhea,nausea,hypoglycemia)were compared between the two groups.Results Compared with 72.73%of the total effective rate of the control group,the total effective rate of the study group was higher,accounting for 96.97%(P<0.05).Compared with the lung function indexes and blood glucose levels after treatment in the control group,FVC,FEV_(1),FEV_(1)/FVC and VC in the study group were higher,and FBG,2 hPG and HbA1c were lower(P<0.05).Compared with 30.30%of the incidence of complications in the control group,the incidence of complications in the study group was lower,accounting for 6.06%(P<0.05).Conclusion Budesonide and formoterol fumarate powder for inhalation combined with tiotropium bromide powder for inhalation is effective in the treatment of stable chronic obstructive pulmonary disease with diabetes,which can improve their lung function,reduce the blood glucose level,and has high safety.
