摘要
目的:探讨吻合器痔上黏膜环切术(PPH)对出口梗阻性便秘患者直肠肛管测压参数及肛门疼痛的影响。方法:选择2022年5月—2024年10月荆州市第三人民医院肛肠外科收治的120例出口梗阻性便秘患者,按照随机数字表法分成两组,每组60例。对照组行传统直肠黏膜柱状缝扎术治疗,观察组行PPH治疗。比较两组临床指标、直肠肛管测压参数、肛门疼痛评分、生活质量评分及并发症发生情况。结果:观察组创口愈合时间早于对照组,排便时间、住院时间短于对照组(P<0.05);两组术中出血量比较,差异无统计学意义(P>0.05)。术后,观察组肛管最大收缩压、肛管舒张压、肛管静息压高于对照组(P<0.05);两组术后直肠静息压比较,差异无统计学意义(P>0.05)。术后,观察组生活质量综合评定问卷(GQOLI-74)各维度评分高于对照组(P<0.05)。术后12、24、72 h,观察组VAS评分均低于对照组(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:采取PPH治疗出口梗阻性便秘患者能够明显减轻其肛门疼痛,保护肛门功能,缩短住院时间,提升生活质量,且术后并发症未明显增加,安全性较高。
Objective:To explore the influence of procedure for prolapse and hemorrhoids(PPH)on anorectal manometry parameters and anal pain in patients with outlet obstructive constipation.Method:A total of 120 patients with outlet obstructive constipation admitted to the Anorectal Surgery Department of Jingzhou Third People's Hospital from May 2022 to October 2024 were selected and divided into two groups according to the random number table method,with 60 cases in each group.The control group was treated with traditional rectal mucosal columnar suture ligation,while the observation group was treated with PPH.The clinical indicators,anorectal manometry parameters,anal pain scores,quality of life scores and occurrence of complications of the two groups were compared.Result:The wound healing time of the observation group was earlier than that of the control group,and the defecation time and hospital stay were shorter than those of the control group(P<0.05).There was no significant difference in intraoperative blood loss between the two groups(P>0.05).Postoperatively,the maximum systolic pressure,diastolic pressure and resting pressure of the anal canal in the observation group were higher than those in the control group(P<0.05).There was no statistically significant difference in the postoperative rectal resting pressure between the two groups(P>0.05).The VAS scores at 12,24 and 72 hours post-operation in the observation group were lower than those of the control group(P<0.05).The scores of each dimension of the generic quality of life inventory-74(GQOLI-74)in the observation group were higher than those in the control group(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:The treatment of patients with outlet obstructive constipation with PPH can significantly alleviate their anal pain,protect anal function,shorten hospital stay,improve quality of life,and the postoperative complications do not increase significantly,with relatively high safety.
