摘要
目的探讨后外侧入路锁定支撑钢板内固定治疗Bartoníček Ⅳ型后踝骨折的疗效。方法采用后外侧入路锁定支撑钢板内固定治疗23例Bartoníček Ⅳ型后踝骨折患者。记录骨折复位质量、骨折愈合时间、踝关节功能恢复情况和并发症发生情况。结果患者均获得随访,时间12~36(15.5±5.4)个月。术后根据Burwell-Charnley标准评价骨折复位质量:解剖复位21例,复位一般2例。骨折均愈合,时间8~16(11.7±2.3)周。切口均一期愈合,术后未发生感染、骨折不愈合、内固定失效和长屈肌挛缩等并发症。末次随访时,AOFAS踝-后足评分71~100(90.9±7.3)分,其中优14例,良8例,可1例,优良率为22/23。结论后外侧入路锁定支撑钢板内固定治疗Bartoníček Ⅳ型后踝骨折具有复位准确、固定牢靠、患者可早期功能锻炼等优点,疗效满意。
Objective To investigate the effect of locking support plate internal fixation in the treatment of Bartoníček type Ⅳposterior malleolar fracture.Methods The 23 patients with posterior malleolar fractures of Bartoníček type Ⅳwere treated with locking support plates fixation via posterolateral approach.The quality of fracture reduction,healing time,ankle functional recovery and complications were recorded.Results All patients were followed up for 12~36(15.5±5.4)months.According to Burwell-Charnley fracture reduction evaluation criteria after operation:Anatomical reduction was in 21 cases and general reduction in 2 cases.All the fractures healed within 8~16(11.7±2.3)weeks.All incisions healed in one-stage,and no complications such as infection,fracture nonunion,failure of internal fixation and flexor longis contracture occurred.At the last follow-up,the ankle-hindfoot score of AOFAS was 71~100(90.9±7.3)points,of which 14 cases were excellent,8 cases were good,and 1 fair,with an excellent-good rate of 22/23.Conclusions For treatment of Bartoníček type Ⅳposterior malleolar fracture,locking support plate internal fixation via posterolateral approach has the advantages of accurate reduction,firm fixation and early functional exercise,and the clinical efficacy is satisfactory.
