摘要
目的探讨经皮骨盆前环内支架固定(INFIX内固定)联合骶髂关节螺钉治疗老年不稳定型骨盆骨折的疗效。方法采用INFIX内固定联合骶髂关节螺钉治疗19例老年不稳定型骨盆骨折患者。记录手术情况、骨折复位及愈合情况、术后并发症发生情况,采用Majeed评分系统评价功能恢复情况。结果患者均获得随访,时间6~18个月。手术时间52~85 min,术中出血量36~72 ml,住院时间3~7 d。采用Matta标准评定术后骨折复位质量:优12例,良5例,可2例。骨痂形成时间4~6(5.07±1.21)周,骨折愈合时间12~16(13.39±1.41)周。术后未发生切口感染、血管损伤、骨折再次移位、内固定物松动或断裂等情况,1例发生股前外皮神经损伤,1例下蹲活动受限。末次随访时Majeed评分65~90(80.12±1.53)分,其中优12例,良5例,可2例,优良率17/19。结论INFIX内固定联合骶髂关节螺钉治疗老年不稳定型骨盆骨折的效果满意,具有机体损伤小、患者术后恢复快、易于接受的优点。
Objective To explore the efficacy of percutaneous anterior pelvic ring internal fixation(INFIX)combined with sacroiliac joint screws in the treatment of unstable pelvic fractures in the elderly.Methods The 19 elderly patients with unstable pelvic fractures were treated with INFIX combined with sacroiliac joint screws.The surgical conditions,fracture reduction and healing,and postoperative complications were recorded.The functional recovery was evaluated using the Majeed scoring system.Results All patients were followed up for 6~18 months.The operation time was 52~85 min,the intraoperative blood loss was 36~72 ml,and the hospital stay was 3~7 d.The postoperative fracture reduction quality was evaluated using the Matta standard:The 12 cases were excellent,5 cases were good,and 2 cases were fair.The bone call formation time was 4~6(5.07±1.21)weeks,and the fracture healing time was 12~16(13.39±1.41)weeks.No incision infection,vascular injury,fracture re-displacement,internal fixation loosening or fracture breakage occurred after surgery.One case had anterior femoral cutaneous nerve injury,and one case had limited squatting activity.At the last follow-up,the Majeed score was 65~90(80.12±1.53)points,including 12 excellent cases,5 good cases,and 2 fair cases.The excellent and good rate was 17/19.Conclusions INFIX combined with sacroiliac joint screws has satisfactory efficacy in the treatment of unstable pelvic fractures in the elderly.It has the advantages of less injury to the body,fast postoperative recovery,and easy acceptance by patients.
