摘要
结合中西医临床病症特点,本文系统梳理了胃癌前病变(precancerous lesions of gastric cancer,PLGC)的动物模型,根据现行临床诊断标准,对其进行评价和临床吻合度分析。目前主流造模方法以N-甲基-N′-硝基-N-亚硝基胍(N-Methyl-N′-nitro-N-nitrosoguanidine,MNNG)致癌剂复合因素联合诱导法为主,具有较高的中西医临床吻合度,但由于诱导因素复杂,可控变量较多,在药物剂量、给药频率、造模周期等方面存在较大差异,考虑与缺乏更为明确统一的阶段性评价指标有关。此外,在传统化学药物诱导基础上,施以气虚证、湿热证相关因素干预,成功模拟脾胃虚弱证、脾胃湿热证等证候类型,体现中医病证结合的临床特点,但由于动物模型在一般行为表现方面与人类临床存在一定差距,因此建议未来制定专门的行为评价量表,完善模型评价体系,以提高PLGC动物模型的临床相关性。
In this paper,the animal models of precancerous lesions of gastric cancer(PLGC)were systematically reviewed based on the characteristics of clinical diseases in traditional Chinese and Western medicine,and evaluated and analyzed for clinical coincidence according to the current clinical diagnostic criteria.At present,the combination of compound factors of a carcinogen,N-Methyl-N′-nitro-N-nitrosoguanidine(MNNG),with induction method is currently the mainstream modeling method,which has a high degree of clinical coincidence in traditional Chinese and Western medicine.However,due to the complexity of induction factors and the large number of controllable variables,there are significant differences in drug dose,administration frequency and modeling cycle,which are considered to be related to the lack of more explicit and unified phased evaluation indicators.In addition,syndromes such as spleen and stomach weakness and spleen-stomach damp-heat have been successfully simulated by intervening with factors related to qi deficiency syndrome and damp-heat syndrome on the basis of traditional chemical drug induction,reflecting the clinical characteristics of the combination of diseases and syndromes in traditional Chinese medicine.However,since there are certain disparities between animal models and humans in general clinical behavioral manifestations,it is recommended to formulate a specialized behavioral evaluation scale and improve the model evaluation system in the future,so as to enhance the clinical relevance of PLGC animal models.
