摘要
乙型肝炎病毒持续感染引发的免疫微环境失衡与纤维化进程仍是临床管理难点。近年来,胞外囊泡(EV)作为一种可携带病毒组分与宿主调控分子的纳米级载体,在慢性乙型肝炎(CHB)疾病进展中的调控作用逐渐成为研究热点。EV通过介导细胞间通信,在介导乙型肝炎病毒组分转移、免疫应答调节及肝星状细胞活化与纤维化形成中发挥作用。本文综述EV在CHB相关免疫调控和肝纤维化中作用机制的最新进展,探讨其作为生物标志物或治疗靶点的潜力,为CHB开发基于EV的新型诊断与治疗策略提供新思路。
The persistent immune microenvironment imbalance and fibrogenesis triggered by hepatitis B virus infection remain major clinical challenges.In recent years,extracellular vesicle(EV)as nanoscale carriers capable of transporting viral components,and host regulatory molecules,have attracted increasing attention for their regulatory roles in the progression of chronic hepatitis B(CHB).EV plays a role in mediating the transfer of hepatitis B virus components,immune response regulation,activation and fibrosis formation of hepatic stellate cells by mediating intercellular communication.This article reviews the latest progress on action mechanism of EV in CHB associated immune regulation and liver fibrosis,explores its potential as a biomarker or therapeutic target,and provides new ideas for the development of novel diagnostic and therapeutic strategies for CHB based on EV.
