摘要
目的 探索氨氯地平与二甲双胍联合用药针对高血压并糖尿病肾病患者的治疗效果。方法 选取2021年2月至2024年11月期间,在本院接受治疗的82例高血压合并糖尿病肾病患者作为研究对象。根据双色球盲分法将患者分为两组,联合组和常规组,各41例。常规组每天口服一次5毫克的苯磺酸氨氯地平片,联合组在常规组治疗的基础上添加盐酸二甲双胍片治疗,连续治疗3个月。观察两组患者的血压指标变化、血糖指标变化、肾功能各项指标的改变状况与不良反应的发生比率。结果 在血压方面,联合组患者的舒张压与收缩压相较于常规组明显降低,差异具有统计学意义(P<0.05)。在血糖指标上,联合组患者的空腹血糖值、餐后2小时血糖值以及糖化血红蛋白均显著低于常规组,差异存在统计学意义(P<0.05)。而在肾功能指标方面,联合组治疗后的血肌酐和尿素氮等指标也明显低于常规组,差异具有统计学意义(P<0.05)。结论 氨氯地平片与二甲双胍的联合应用在治疗糖尿病肾病方面不仅能够更有效地控制血压和血糖水平,还能够显著改善患者的肾功能,减少肾脏损害,该治疗方案值得在临床中进一步推广和应用。
Objectiveexplore the therapeutic efficacy of the combined administration of amlodipine and metformin in patients with concurrent hypertension and diabetic nephropathy.Methods A total of 82 patients diagnosed with hypertension complicated by diabetic nephropathy who received treatment at our hospital from February 2021 to November 2024 were selected as the study cohort.Using a double-blind randomization method,patients were divided into two groups:the combination group(n=41)and the conventional group(n=41).The conventional group received oral administration of 5 mg amlodipine besylate tablets once daily,while the combination group received the same amlodipine regimen supplemented with metformin hydrochloride tablets.The treatment duration was 3 months.Blood pressure parameters,glycemic indices,renal function markers,and the incidence of adverse reactions were monitored in both groups.ResultsIn terms of blood pressure control,the combination group demonstrated significantly lower diastolic and systolic blood pressure values compared to the conventional group(P<0.05).Regarding glycemic management,the combination group exhibited markedly reduced fasting plasma glucose,2-hour postprandial glucose,and glycated hemoglobin(HbA1c)levels relative to the conventional group(P<0.05).Additionally,renal function assessments revealed significantly lower serum creatinine and blood urea nitrogen levels in the combination group post-treatment(P<0.05).Conclusion The combined use of amlodipine and metformin not only achieves more effective blood pressure and glycemic control in patients with diabetic nephropathy but also significantly improves renal function and reduces kidney damage.This therapeutic regimen warrants further clinical promotion and application.
