CD34, GS和Glypican-3在丙型肝炎病毒相关肝癌组织中表达及与预后关系

Expression of CD34,GS,and Glypican-3 in HCV-related hepatocellular carcinoma tissues and their relationship with prognosis

摘要 目的 探讨CD34、谷氨酰胺合成酶(GS)和磷脂酰肌醇蛋白聚糖-3(Glypican-3)在丙型肝炎病毒(HCV)相关肝癌组织中的表达及与预后关系。方法 回顾性选取2018年1月至2023年12月收治入川北医学院附属医院的丙型肝炎相关性肝细胞癌(HCC)患者86例为研究组,另采用倾向性匹配评分法选取同期于川北医学院附属医院就诊的丙型肝炎相关性肝硬化患者80例为对照组。采用Max Vision法检测两组患者肝癌和肝硬化组织中GS、CD34、Glypican-3蛋白表达并比较两组的阳性表达率。对研究组进行1年随访,术后复发35例,纳入不良组,另51例患者未发生复发纳入良好组。分析并比较不良组与良好组的GS、CD34、Glypican-3蛋白表达及其与病理特征的关系。结果 研究组GS、CD34、Glypican-3蛋白表达阳性率均高于对照组(χ^(2)=89.578,30.379,46.495,P<0.05)。TNM分期为Ⅲ-Ⅳ期和低分化程度的患者GS、CD34、Glypican-3蛋白表达阳性率高于TNM分期为Ⅰ-Ⅱ期和中、高分化程度的患者(P<0.05)。不良组Ⅲ-Ⅳ期、低分化以及GS、CD34、Glypican-3蛋白表达阳性占比高于良好组(χ^(2)=7.300,43.466,6.831,33.055,11.649,P<0.05)。结论 在丙型肝炎相关性HCC肝癌组织中,GS、CD34、Glypican-3呈现蛋白阳性表达,与TNM分期和分化程度相关;TNM分期、分化程度、GS蛋白表达、CD34蛋白表达、Glypican-3蛋白表达是丙型肝炎相关性HCC患者术后复发的影响因素。 Objective To investigate the expression of CD34,glutamine synthetase(GS),and glypican-3 in hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC)tissues and their relationship with prognosis.Methods Eighty-six patients with HCV-related HCC admitted to our hospital from January 2018 to December 2023 were retrospectively enrolled in the study group.Additionally,80 patients with HCV-related liver cirrhosis who visited our hospital during the same period were selected as the control group using propensity score matching.The MaxVision method was used to detect the expression of GS,CD34,and glypican-3 proteins in HCC tissues and cirrhotic tissues,and the positive expression rates were compared between the two groups.The study group was followed up for one year:35 patients with postoperative recurrence were assigned to the poor prognosis group,and 51 patients without recurrence were assigned to the good prognosis group.The expression of GS,CD34,and glypican-3 proteins,as well as their associations with pathological features,were analyzed and compared between the poor and good prognosis groups.Results The positive expression rates of GS,CD34,and glypican-3 proteins in the study group were significantly higher than those in the control group(χ^(2)=89.578,30.379,46.495,all P<0.05).Patients with TNM stage III-IV and poorly differentiation tumors had higher positive expression rates of GS,CD34,and glypican-3 proteins than those with TNM stage I-II and moderately/well-differentiated tumors(all P<0.05).Compared with the good prognosis group,the poor prognosis group had a higher proportion of patients with TNM stage III-IV,poorly differentiated tumors,and positive expression of GS,CD34,and glypican-3 proteins(χ^(2)=7.300,43.466,6.831,33.055,11.649,P<0.05).Conclusion In HCV-related HCC tissues,GS,CD34,and glypican-3 exhibit positive expression,which is related to TNM staging and tumor differentiation.TNM staging,tumor differentiation,and the expression of GS,CD34,and glypican-3 proteins are influencing factors for postoperative recurrence in patients with HCV-related HCC.
