摘要
目的 通过对比分析对口帮带医疗帮带前后,尼玛县人民医院救护车向上转诊患者数据,评估医疗帮带对该院医疗业务的影响,以期为高原基层医院的业务发展、学科建设提供实证支持和政策建议。方法 以2022年6月至12月对口帮带时期作为空窗期,收集并分析了该时间节点前后,门急诊救护车向上转诊患者相关数据,运用统计学方法,对患者数量、病种、转运分型以及患者预后等指标进行深入分析比较。结果 在医疗帮带后,急性阑尾炎、胆囊结石和某些慢性病患者转诊比例显著下降(P=0.001),而外伤患者和其他急症患者转诊比例较帮带前有所增加。基础型、护送型和医疗型转诊比例有所下降,重症型转诊比例有所上升(P=0.028)。结论 对口医疗帮带对医院的整体医疗业务能力产生了显著的正面影响,儿科、危重症等专业仍需进一步加强。这些发现为高原地区基层医院的发展提供了宝贵的实证支持,并为未来的政策制定提供了指导性建议。
Objective To evaluate the impact of paired healthcare assistance on hospital operations by comparing upward referral data of ambulance patients at Nyima County People's Hospital before and after the im-plementation of the assistance program,aiming to provide empirical evidence and policy recommendations for en-hancing clinical services and disciplinary development in high-altitude primary hospitals.Methods This study used the period of paired healthcare assistance from June to December 2022 as the window period,collected and analyzed relevant data on patients referred by outpatient and emergency ambulances before and after this period,and used statistical methods to conduct in-depth analysis and exploration of indicators such as patient number,dis-ease types,transport types,and patient prognosis.Results After paired healthcare assistance,the referral rate for patients with acute appendicitis,gallstones,and certain chronic diseases significantly decreased(P=0.0014),while the referral rate for trauma patients and other emergency patients increased.The referral rates for basic,escort,and medical types decreased,while that for severe cases increased(P 0.028).Conclusions Paired healthcare assistance has had a significant positive impact on the overall medical business capabilities of hospitals,but further strengthening is stll needed in specialties such as pediatrics and critical care.These findings provide valuable empirical support for the development of primary hospitals in high-altitude areas and offer guiding suggestions for future policy formulation.
