摘要
目的 调查海军陆战队某部官兵偏颇体质在睡眠与口腔疾病间的中介效应,为促进官兵口腔健康提供中医体质学理论依据。方法 采用口腔疾病调查问卷、中医体质量表以及匹兹堡睡眠质量指数量表(PSQI)对海军陆战队官兵进行口腔健康、中医体质以及睡眠评估,分析三者间的关联性,并进行中介作用分析。结果 纳入的701位官兵中唇炎419例,颞下颌关节紊乱病300例,牙周病296例,复发性口腔溃疡162例,睡眠质量差(PSQI得分>7分)198人(28.25%),偏颇体质314人(44.79%)。单因素logistic回归分析显示睡眠异常是口腔疾病和偏颇体质的危险因素(P<0.05),偏颇体质为4种口腔疾病的危险因素(P<0.05);中介分析表明睡眠异常通过诱发海军陆战队官兵形成偏颇体质间接影响口腔疾病发生(P<0.05)。结论 偏颇体质在睡眠与口腔疾病间存在中介效应,口腔军事卫勤保健中也应关注官兵全身健康,调理中医体质并改善睡眠。
Objective To investigate the mediating effect of biased constitution on the relationship between sleep quality and oral diseases in Marine Corps,so as to provide theoretical basis of traditional Chinese medicine(TCM)constitution for promoting oral health of Marine Corps.Methods The designed oral diseases questionnaire,TCM constitution scale and Pittsburgh Sleep Quality Index(PSQI)were used to evaluate oral health status,TCM constitution identification and sleep quality.The correlation among the three indexes and the mediating effect were analyzed.Results Among the 701 officers and soldiers included,there were 419 cases of cheilitis,300 cases of temporomandibular joint disorder,296 cases of periodontal disease,162 cases of recurrent oral ulcer,198 cases(28.25%)with poor sleep quality(PSQI score>7),and 314 cases(44.79%)with biased constitution.Univariate logistic regression analysis indicated that sleep disorder was a risk factor for oral diseases and biased constitution(P<0.05),and biased constitution was a risk factor for four oral diseases(P<0.05).Mediation analysis showed that sleep disorder indirectly affected the incidence of oral diseases by inducing the formation of biased constitution in Marine Corps(P<0.05).Conclusion Biased constitution has a mediating effect between sleep quality and oral diseases,and the overall health of officers and soldiers should be paid attention to in military oral health care,so as to regulate TCM constitution and improve sleep quality.
