摘要
目的:探究超声下细针穿刺改良小针刀治疗腕管综合征(CTS)的效果。方法:回顾性选取2020年1月—2023年12月在雅安市中医医院进行诊治的79例CTS患者纳入研究。根据治疗方法将其分为对照组和观察组,对照组(40例)予以传统小针刀治疗,观察组(39例)予以超声引导下细针穿刺改良小针刀治疗;两组患者术后均增加弥可保穴位注射治疗。比较两组患者疗效、视觉模拟评分(VAS)、Levine腕管综合征症状及功能问卷(Levine)评分、腕关节活动度、神经肌电指标[正中神经感觉神经传导速度(SCV)、复合肌肉动作电位(CMAP)波幅、运动神经末端运动潜伏期(DmL)]及安全性。结果:观察组疗效优于对照组(P<0.05)。治疗1周、3周,两组VAS评分均较治疗前下降(P<0.05),且观察组治疗1周、3周时VAS评分低于对照组(P<0.05)。治疗后,两组SCV、CMAP波幅均增加(P<0.05),Levine评分、DmL均降低(P<0.05),观察组SCV、CMAP波幅高于对照组(P<0.05),Levine评分、DmL低于对照组(P<0.05)。治疗后,两组背伸、掌屈、桡偏、尺偏活动度均增加(P<0.05),观察组高于对照组(P<0.05)。结论:CTS患者予以超声下细针穿刺改良小针刀治疗,可通过缓解疼痛、改善临床症状及神经肌肉功能,有效提升疗效,从而促进腕关节功能恢复。
Objective:To investigate the efficacy of ultrasound-guided fine-needle puncture modified acupotomy in treating carpal tunnel syndrome(CTS).Methods:A retrospective analysis was conducted on 79 CTS patients treated at Ya an Hospital of Traditional Chinese Medicine from January 2020 to December 2023.Patients were divided into a control group(n=40,treated with traditional acupotomy)and an observation group(n=39,treated with ultrasound-guided fine-needle puncture modified acupotomy).Both groups received additional mecobalamin acupoint injection postoperatively.Treatment efficacy,visual analog scale(VAS)scores,Levine carpal tunnel syndrome questionnaire scores,wrist joint range of motion,neuroelectrophysiological parameters[sensory nerve conduction velocity(SCV)of the median nerve,compound muscle action potential(CMAP)amplitude,distal motor latency(DML)],and safety were compared between the two groups.Results:The observation group showed superior therapeutic efficacy compared to the control group(P<0.05).At 1 and 3 weeks post-treatment,VAS scores decreased significantly in both groups(P<0.05),with lower scores observed in the observation group(P<0.05).After treatment,SCV and CMAP amplitude increased while Levine scores and DML decreased in both groups(P<0.05),with more significant improvements in the observation group(P<0.05).Wrist joint range of motion(dorsiflexion,palmar flexion,radial deviation,ulnar deviation)improved in both groups(P<0.05),with greater improvement in the observation group(P<0.05).Conclusion:Ultrasound-guided fine-needle puncture modified acupotomy effectively relieves pain,improves clinical symptoms and neuromuscular function in CTS patients,thereby enhancing therapeutic efficacy and promoting wrist joint functional recovery.
