摘要
目的:探究济生肾气丸辅助西医治疗对肾阳虚亏证老年良性前列腺增生(BPH)的应用效果。方法:选取2023年1月—2024年5月江苏省人民医院宿迁医院收治的180例老年BPH患者,根据治疗方法分为西医组、中医组和联合组,每组60例,西医组予以坦索罗辛+非那雄胺治疗,中医组予以济生肾气丸治疗,联合组予以中西医结合治疗。比较3组临床疗效、血清指标[肿瘤坏死因子-α(TNF-α)、白细胞介素(IL-6),干扰素-γ(INF-γ)、睾酮(T)、雌二醇(E_(2))、前列腺特异抗原(PSA)],尿流动力学指标[最大尿流率(Qmax)、膀胱残余尿量(RUV)],前列腺体积(PV),国际前列腺症状评分(IPSS),匹茨堡睡眠质量指数(PSQI)。结果:联合组疗效优于西医组和中医组(P<0.05)。治疗后,3组主症及次症积分均降低(P<0.05),联合组和中医组均低于西医组(P<0.05)。治疗后,3组TNF-α、IL-6、E_(2)/T、PSA、PV、IPSS评分、RUV、PQSI评分减少(P<0.05),联合组低于中医组和西医组(P<0.05),3组Qmax增加(P<0.05),联合组高于中医组和西医组(P<0.05)。结论:济生肾气丸与坦索罗辛、非那雄胺联合用于肾阳虚亏证老年BPH的临床治疗,可有效减轻炎症、调节性激素水平,改善前列腺下尿路症状及尿流动力学情况,从而有效提升疗效,改善生活质量。
Objective:To investigate the therapeutic effect of Jisheng Shenqi Pills as adjuvant to Western medicine in elderly patients with benign prostatic hyperplasia(BPH)of kidney-yang deficiency syndrome.Methods:A total of 180 elderly BPH patients admitted to Suqian Hospital of Jiangsu Provincial People's Hospital from January 2023 to May 2024 were selected and divided into three groups(n=60 each)according to treatment methods:Western medicine group(tamsulosin+finasteride),traditional Chinese medicine(TCM)group(Jisheng Shenqi Pills alone),and combination group(integrated Chinese-Western therapy).Therapeutic efficacy,serum indicators[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interferon-γ(INF-γ),testosterone(T),estradiol(E2),prostate-specific antigen(PSA)],urodynamic parameters[maximum urinary flow rate(Qmax),residual urine volume(RUV)],prostate volume(PV),International Prostate Symptom Score(IPSS),and Pittsburgh Sleep Quality Index(PSQI)were compared.Results:The combination group showed superior efficacy compared to the Western medicine and TCM groups(P<0.05).After treatment,the scores of primary and secondary symptoms decreased in all three groups(P<0.05),with lower scores in the combination and TCM groups than in the Western medicine group(P<0.05).Post-treatment,TNF-α,IL-6,E2/T ratio,PSA,PV,IPSS,RUV,and PSQI decreased in all groups(P<0.05),with the combination group exhibiting lower values than the TCM and Western medicine groups(P<0.05).Qmax increased in all groups(P<0.05),with the highest improvement in the combination group(P<0.05).Conclusion:The combination of Jisheng Shenqi Pills with tamsulosin and finasteride effectively reduces inflammation,regulates sex hormone levels,improves lower urinary tract symptoms and urodynamic parameters in elderly BPH patients with kidney-yang deficiency,thereby enhancing therapeutic efficacy and quality of life.
