摘要
目的调查上海市松江区急性缺血性脑卒中(AIS)患者发病后运送到达医院就诊的时间、溶栓、康复情况,分析脑卒中健康管理对就诊时间、溶栓、神经功能的影响,并对松江区公共卫生体系建设项目脑卒中健康管理的开展提出建议。方法我院自2022年6月开始启动脑卒中健康管理,将2020年6月-2022年5月住院的AIS患者设为对照组,2022年6月-2024年5月住院AIS患者设为观察组,比较二组AIS患者的人次数、卒中相关知识知晓率、120使用、就诊时间、溶栓率、14 d NIHSS评分和mRS评分。结果观察组住院人次、卒中知识知晓率、120使用率、溶栓率较对照组增多,就诊时间较对照组缩短,统计学意义显著(P<0.01)。观察组治疗后NIHSS评分和mRS评分较对照组低,统计学意义显著(P<0.01)。结论开展脑卒中健康管理可增加卒中早期就诊率,缩短就诊时间,提升溶栓率,改善神经功能。脑卒中健康管理还要加强卒中危险因素的管理,做到关口前移,加强医院与院前急救系统的密切合作,完善医院急救体系,强化急诊医生救治技能。
Objective To investigate the time of arrival,thrombolysis and rehabilitation of patients with acute ischemic stroke(AIS)in Songjiang District of Shanghai,to analyze the impact of stroke health management on the time of visit,thrombolysis and neurological function,and to provide suggestions for the development of stroke health management in the public health system construction project in Songjiang District.Methods Stroke health management was initiated from June 2022 in our hospital.AIS patients hospitalized from June 2020 to May 2022 were set as the control group,and AIS patients hospitalized from June 2022 to May 2024 were set as the observation group.The number of AIS patients,the awareness rate of stroke-related knowledge,120 use,treatment time,thrombolysis rate,14-day NIHSS score and mRS score were compared between the two groups.Results The number of hospitalizations,awareness rate of stroke knowledge,120 utilization rate and thrombolysis rate in the observation group were significantly higher than those in the control group,and the treatment time was shorter than that in the control group,with statistical significance(P<0.01).After treatment,the NIHSS score and mRS score of the observation group were lower than those of the control group,and the statistical significance was significant(P<0.01).Conclusion Stroke health management can increase the rate of early treatment of stroke,shorten the time of treatment,increase the rate of thrombolysis,and improve neurological function.Stroke health management should also strengthen the management of stroke risk factors,move forward,strengthen the close cooperation between the hospital and the pre-hospital emergency system,improve the hospital emergency system,and strengthen the emergency doctor's treatment skills.
