摘要
目的探索县级医疗机构全科临床药师继续教育培训模式,为更好地开展基层临床药师培养提供依据。方法检索2014年1月—2024年1月,国内外全科临床药师继续教育培训的相关文献,通过文献筛选、小组讨论等方式提取能力评价指标,初步构建培训体系框架,采用界值法对指标进行筛选。应用问卷星软件,基于德尔菲法开展问卷调查,评估专家对该体系指标的一致性和可靠性。结果国内6个省份39家县级医疗机构的40名药学专家参与本研究,两轮问卷调查回收率分别为100%和97.5%,表明专家参的积极性高。通过判断系数和熟悉度,得出专家权威系数为0.87。最终形成的指标体系包含4个一级指标和22个二级指标,肯德尔协调系数介于0.089~0.182之间,P值均小于0.05,表明经过两轮询函,专家意见一致性良好,结果可靠。结论本研究可为基层紧缺的全科临床药师的继续教育和能力评估提供参考。
Objective To explore the continuing education and training model for general clinical pharmacists in county-level medical institutions,and to provide a basis for better training of grassroots clinical pharmacists.Methods Relevant literature on continuing education and training for general clinical pharmacists both domestically and internationally from January 2014 to the present was reviewed.Competency evaluation indicators were extracted through literature screening and group discussions,and a preliminary training system framework was constructed.The boundary value method was used to screen the indicators.A questionnaire survey based on the Delphi method was conducted using Questionnaire Star software to evaluate the consistency and reliability of the experts'assessments of the system indicators.Results Forty pharmaceutical experts from 39 county-level medical institutions across six provinces in China participated in this study.The response rates for the two rounds of questionnaires were 100%and 97.5%,respectively,indicating a high level of expert engagement.The expert authority coefficient,determined by the judgment coefficient and familiarity,was 0.87.The final indicator system included 4 first-level indicators and 22 second-level indicators.The Kendall’s coordination coefficients ranged from 0.089 to 0.182,with all P-values less than 0.05,indicating good consensus among experts after two rounds of surveys and reliable results.Conclusion This study can serve as a reference for the continuing education and competency evaluation of general clinical pharmacists,who are in high demand at the grassroots level.
