基于物联网的可穿戴心电设备在心律失常实时监测中的价值及三级慢病管理模式研究

The Value of Wearable Electrocardiogram Devices based on the Internet of Things in Real-time Monitoring of Arrhythmia and Research on the Three-level Chronic Disease Management Mode

摘要 目的基于物联网技术,研究可穿戴心电监测设备在心律失常高危人群中的监测价值,探讨慢病管理新模式。方法选取2023年4月—2024年4月,在天津市第三中心医院就诊的324例心血管高危因素患者为研究对象,利用AliveCor可穿戴设备进行实时心电监测,建立“症状触发式心电监测+医生端实时诊断”模式,并与常规心电图与Holter的进行对比,评估患者满意度。结果移动心电设备对窦性心动过速的检测效能明显优于常规心电图和Holter、在ST-T改变、房性早搏、室性早搏、房颤、QRS时间延长、等几类心律失常的检测效能显著优于常规心电图,与Holter检测效能无差异。患者满意度调查显示,94.75%的患者认可移动心电监测模式,其中就医便捷性满意度最高,但报告时效性与医患沟通有待改善。结论可穿戴心电监测设备可显著提升偶发性心律失常的早期筛查效率,通过构建“居家—社区—医院”三级协同管理模式,促进可穿戴设备在三级慢病管理中的推广和应用。 Objective To study the monitoring value of wearable ECG monitoring devices in high-risk groups of arrhythmias based on Internet of Things technology,and to explore a new model of chronic disease management.Methods From April 2023 to April 2024,324 patients with cardiovascular risk factors in a tertiary hospital in Tianjin were selected as the research subjects,and the AliveCor wearable device was used for real-time ECG monitoring,and the“symptom-triggered ECG monitoring and doctor-side real-time diagnosis”mode was established,and the patient satisfaction was evaluated by comparing it with conventional ECG and Holter.Results The detection efficiency of mobile ECG equipment for sinus tachycardia was significantly better than that of conventional electrocardiogram and Holter,ST-T,and atrial premature contractions,premature ventricular contractions,atrial fibrillation,prolonged QRS time,and other types of arrhythmias were significantly better than those of conventional electrocardiogram,which was no different from the detection performance of Holter.The patient satisfaction survey showed that 94.75%of the patients agreed with the mobile ECG monitoring mode,among which the convenience of medical treatment was the highest,but the timeliness of reporting and doctor needed to be improved.Conclusion Wearable ECG monitoring devices can significantly imProve the efficiency of early screening of episodic arrhythmias,and promote the promotion and application of wearable devices in the management of tertiary chronic diseases by constructing a“home-community-hospital”three-level collaborative management model.
