摘要
目的评估临床路径在牙槽嵴低平患者全口义齿修复中的应用效果。方法回顾性分析2019年10月—2024年10月天津市南开医院口腔科收治的130例牙槽嵴低平牙列缺失患者资料。根据治疗路径不同,分为临床路径组和传统修复组,每组65例患者。两组患者在治疗后每半年进行一次复查,比较指标包括:义齿调整次数、咀嚼效能(吸光度值)、咬合力、咬肌肌电值不对称指数、主观咀嚼功能、基托活动度、食物嵌塞率。结果临床路径组咀嚼效能吸光度值、咬合力显著高于传统修复组,双侧咬肌肌电值不对称指数低于传统修复组。临床路径组患者主观咀嚼功能改善率显著高于传统修复组。此外,临床路径组的适应期义齿调整次数、基托存在肉眼可见活动度的比例、食物嵌塞率均低于传统修复组,这些差异均具有统计学意义(P<0.05)。结论在牙槽嵴低平患者全口义齿修复中采用临床路径,可显著提升修复质量。
Objective To evaluate the application effect of clinical pathway in the restoration of complete edentulism for patients with low alveolar ridges.Methods A retrospective analysis was conducted on the medical records of 130 patients with edentulism and low alveolar ridge treated at the Department of Stomatology of Nankai Hospital,Tianjin,from October 2019 to October 2024.Patients were divided into two groups based on different treatment pathways:the clinical pathway group and the traditional restoration group,with 65 patients in each group.Patients in both groups underwent follow-up examinations every six months post-treatment.Comparison indicators included:number of denture adjustments,masticatory efficiency(absorbance value),bite force,masseter muscle electromyography asymmetry index,subjective masticatory function,denture base mobility,and food impaction rate.Results The clinical pathway group had significantly higher chewing efficiency absorbance values and bite force than the traditional restoration group,while the bilateral masseter muscle electromyography asymmetry index was lower than that of the traditional restoration group.The improvement rate in subjective masticatory function was significantly higher in the clinical pathway group than in the traditional restoration group.Additionally,the clinical pathway group had fewer adjustments to the trial denture during the adaptation period,a lower proportion of denture bases with visible mobility,and a lower food impaction rate than the traditional restoration group.These differences were statistically significant(P<0.05).Conclusion The use of a clinical pathway in complete denture restoration for patients with low alveolar ridge can significantly improve restoration quality.
