Gibbs反思循环圈视角下新生儿重症监护室(NICU)护理本科实习生的反思性实践体验质性研究

A Qualitative Study on Reflective Practice Experience of Undergraduate Nursing Interns in Neonatal Intensive Care Unit(NICU)from the Perspective of Gibbs Reflective Circle

摘要 目的通过分析护理本科实习生在新生儿重症监护室(Neonata1 Intensive Care Unit,NICU)实习期间所撰写的反思日志,深入探讨其反思性实践体验,识别共性问题,并为优化实习方案提供理论依据。方法采用质性研究中现象学研究法,对24名NICU实习护士的反思日志研究,并运用Colaizzi方法对资料分析。结果提炼出三个核心主题:理论-实践差距感知(包括专科操作挑战、隐性护理知识的渗透、教育和培训的差异)、负性心理体验(如高度紧张、自我效能感低、对反馈的恐惧),以及反思性实践对护生产生的积极体验(临床思维的转变、职业认同感的提升、自我期望的增强)。结论护理本科实习生在NICU临床实践中感知到理论教学与临床实践之间的差异,并容易出现负性心理体验。建议加强教学队伍建设,实施隐性知识共享,开展循证护理教学,将科研证据、丰富的护理临床经验和患者需求相结合,积极引导学生投身临床实践,以有效避免理论与实践的脱节,并关注学生的心理健康。同时,开展反思性实践教学,逐步形成符合NICU护生培养需求的护理教学体系。 Objective To analyze the reflective journals written by undergraduate nursing interns during their neonatal intensive care unit(NICU)internship,to explore in depth their reflective practice experiences,to identify common issues,and to provide a theoretical basis for optimizing the internship program.Methods Using the phenomenological qualitative research method,the reflective journals of 24 NICU student nurses were read in depth,and the data were analyzed using the Colaizzi method.Results Three core themes were extracted:perceived theory-practice gap(including specialty operational challenges,penetration of tacit nursing knowledge,and differences in education and training),negative psychological experiences(e.g.,high levels of nervousness,low self-efficacy,and fear of feedback),and positive experiences of reflective practice for nursing students(shifts in clinical thinking,increased professional identity,and enhanced self-expectations).Conclusions Undergraduate nursing students perceive differences between theoretical teaching and clinical practice in NICU clinical practice and are prone to negative psychological experiences.Therefore,it is recommended to strengthen the construction of the teaching team,implement tacit knowledge sharing,conduct evidence-based nursing teaching,combine scientific research findings,rich nursing clinical experience,and patients’needs,and actively guide students into clinical practice to effectively avoid the gap between theory and practice and pay attention to students'psychological health.At the same time,reflective practice teaching is carried out to gradually build a nursing teaching system that meets the training needs of NICU nursing students.
