摘要
目的探讨盐酸氨溴索联合布地奈德对慢性鼻窦炎伴鼻息肉术后患者的临床效果。方法研究以本院慢性鼻窦炎伴鼻息肉患者作为研究样本,共筛选60例,收集时间点:2023年9月至2024年9月,分为对照组和观察组各30例。对照组接受鼻窦内镜手术治疗及术后常规治疗,观察组术后额外给予盐酸氨溴索联合布地奈德雾化吸入治疗。统计分析两组治疗前、治疗3个月后症状及体征评分(SNOT-20评分、Lund-Kennedy评分及VAS评分)、鼻黏膜纤毛清除功能指标[鼻黏膜纤毛清除率和清除速度、糖精消除时间]、相关临床观察指标[嗜酸性粒细胞、杯状细胞数及上皮下腺体计数、上皮损伤分级、细胞外基质沉积(条带样增生厚度)]、生活质量;记录患者临床疗效、半年复发率及用药期间不良反应。结果观察组症状评分、SNOT-20评分、Lund-Kennedy评分及VAS评分均低于对照组(P<0.05),糖精消除时间短于对照组(P<0.05),鼻黏膜纤毛清除率和清除速度均高于对照组(P<0.05),嗜酸性粒细胞、杯状细胞数及上皮下腺体计数、上皮损伤分级、细胞外基质沉积(条带样增生厚度)、生活质量评分均低于对照组(P<0.05),优良率高于对照组(P<0.05),半年复发率低于对照组(P<0.05),但两组用药期间不良反应发生率比较差异不显著(P>0.05)。结论氨溴索联合布地奈德能显著提高慢性鼻窦炎伴鼻息肉患者术后疗效,有效促进鼻黏膜重塑,为临床制定此类患者的治疗方案提供参考依据。
Objective To explore the clinical efficacy of ambroxol hydrochloride combined with budesonide in patients with chronic sinusitis and nasal polyps after surgery.Methods The patients with chronic sinusitis and nasal polyps in our hospital were selected as the research sample.A total of 60 cases were screened,and the collection time points were from September 2023 to September 2024.The patients were divided into the control group and the observation group,with 30 cases in each group.The control group received endoscopic sinus surgery and postoperative routine treatment,while the observation group received additional nebulized inhalation therapy with ambroxol hydrochloride and budesonide after surgery.Statistical analysis was conducted on the symptom and sign scores(SNOT-20 score,Lund-Kennedy score,and VAS score),nasal mucosal ciliary clearance function indicators[nasal mucosal ciliary clearance rate and clearance speed,saccharin elimination time],related clinical observation indicators[eosinophils,goblet cells,subepithelial gland count,epithelial damage grade,extracellular matrix deposition(strip-like hyperplasia thickness)],and quality of life;the clinical efficacy,half-year recurrence rate,and adverse reactions during medication were recorded for the patients.Results The symptom scores,SNOT-20 scores,Lund-Kennedy scores,and VAS scores of the observation group were lower than those of the control group(P<0.05),the saccharin elimination time was shorter than that of the control group(P<0.05),the nasal mucosal ciliary clearance rate and clearance speed were higher than those of the control group(P<0.05),the eosinophils,goblet cells,subepithelial gland count,epithelial damage grade,extracellular matrix deposition(strip-like hyperplasia thickness),and quality of life scores of the observation group were lower than those of the control group(P<0.05),the clinical efficacy rate was higher than that of the control group(P<0.05),the half-year recurrence rate was lower than that of the control group(P<0.05),but there was no significant difference in the incidence of adverse reactions during medication between the two groups(P>0.05).Conclusion Ambroxol hydrochloride combined with budesonide can significantly improve the postoperative efficacy of patients with chronic sinusitis and nasal polyps,effectively promote nasal mucosal remodeling,and provide a reference basis for the clinical formulation of treatment plans for such patients.
