青蒿素及其衍生物治疗肺动脉高压研究进展

摘要 肺动脉高压(pulmonary hypertension,PH)是指由多种病因导致的以肺动脉压力升高、肺血管重塑为核心特征的临床综合征。肺动脉高压病理机制复杂,涉及多种信号通路的异常激活及相互作用。作为一类高效低毒性的抗疟疾药物,青蒿素及其衍生物近年来在肺动脉高压治疗研究中受到重视。现有研究表明,青蒿素类药物可通过舒张肺血管、改善内皮功能、抑制平滑肌增殖与炎症反应等多重途径改善肺血管重构、降低肺动脉压力。值得关注的是,当前研究主要基于动物实验和细胞模型层面的探索,其大规模临床数据仍需进一步充实。基于此,未来研究应着力构建个体化给药方案,建立长期安全性监测框架,并通过多学科协作推进临床转化进程,为肺动脉高压治疗探索新路径。 Pulmonary hypertension(PH)is a clinical syndrome characterized by elevated pulmonary arterial pressure and pathological pulmonary vascular remodeling,resulting from diverse etiological factors.Its pathological mechanisms could involve complex interactions of abnormally activated signaling pathways.Artemisinin and its derivatives,recognized as potent and low-toxicity antimalarial agents,have emerged as promising candidates for PH therapeutic research in recent years.Current evidence confirms that these compounds ameliorate pulmonary vascular remodeling and reduce pulmonary arterial pressure through various pathways,including pulmonary vasodilation,endothelial function enhancement,inhibition of smooth muscle proliferation,and suppression of inflammatory responses.However,previous studies predominantly focused on animal experiments and cellular-level models.Large-scale clinical data-based studies are needed for further validation.Consequently,future investigations should focus on developing personalized dosage regimens,establishing long-term safety monitoring systems,and promoting clinical translation through multidisciplinary collaboration,aiming to explore novel therapeutic avenues for pulmonary hypertension.
