摘要
目的 探讨基于跨理论模型(TTM)的个案管理在胃肠息肉切除患者围术期的运用效果。方法 选取医院在2022年1月-2024年1月收治的120例胃肠息肉切除患者为研究对象,随机将其分为观察组和对照组,每组60例。对照组在围术期给予常规管理,观察组在围术期给予TTM的个案管理,比较两组患者术后恢复情况、焦虑自评量表(SAS)、抑郁自评量表(SDS)、自护能力评分量表(ESCA)、并发症发生率。结果 观察组住院、下床活动、胃肠功能恢复时间均短于对照组,差异有统计学意义(P<0.05)。观察组干预后SAS、SDS评分均低于对照组,差异有统计学意义(P<0.05)。观察组干预后ESCA 4项评分高于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论 TTM的个案管理在胃肠息肉切除患者围术期中的应用,可以改善患者的康复效果、心理状态、自我管理能力,减少并发症的发生率,为患者提供更全面、个性化的医疗护理服务。
Objective To explore the effectiveness of case management based on the Transdisciplinary Theory Model(TTM)in the perioperative period for patients with the gastrointestinal polypectomy.Methods A total of 120 patients with the gastrointestinal polypectomy admitted to the hospital from January 2022 to January 2024 were selected as the study subjects and randomly divided into an observation group and a control group,with 60 patients in each group.The control group received conventional management during the perioperative period,the observation group received TTM-based case management.The postoperative recovery outcomes,Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS),Self-Maintenance Ability Scale for Adults(ESCA)scale,and incidence of complications were compared between the two groups.Results The observation group had shorter hospital stays,ambulation,and time to gastrointestinal function recovery compared to the control group,with statistically significant differences(P<0.05).The observation group had lower SAS and SDS scores after intervention compared to the control group,with statistically significant differences(P<0.05).The ESCA scores in the observation group were higher than those in the control group after intervention,with statistically significant differences(P<0.05).The overall incidence of complications in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).Conclusion The application of TTM case management in the perioperative period of patients undergoing gastrointestinal polypectomy can improve patients'rehabilitation outcomes,psychological status,and self-management abilities,reduce the incidence of complications,and provide comprehensive and personalised medical nursing services.
