摘要
随着人口老龄化趋势加重以及糖尿病患病率的不断上升,老年2型糖尿病(T2DM)患者的出院准备度成为医疗护理领域关注的重点。本研究围绕老年T2DM患者出院准备度的概念内涵、影响因素、评估工具、干预策略进行系统综述,旨在为构建以患者为中心的院外管理路径提供循证依据,并为后续研究与临床实践提供参考,从而进一步提高糖尿病患者健康水平。
With the continuous increase in the aging population and the prevalence of diabe⁃tes,the discharge readiness of elderly patients with type 2 diabetes has become a focus of attention in the medical and nursing fields.This article reviews the research progress on discharge readiness in perspectives of conception,influencing factors,assessment tools and intervention strategies,with the aim of providing a theoretical basis and practical guidance for the discharge preparation of elderly patients with type 2 diabetes mellitus.
