摘要
目的探讨中药内服联合穴位贴敷辅助高效抗逆转录病毒治疗(HAART)对人类免疫缺陷病毒感染/获得性免疫缺陷综合征(HIV/AIDS)患者免疫重建的影响,并分析其影响因素。方法将90例患者随机分为对照组(n=45)和试验组(n=45)。对照组给予HAART,试验组给予HAART联合中药内服及穴位贴敷,疗程1个月。比较两组治疗后1、3、6个月的免疫功能指标与肝肾功能,并通过Logistic回归分析免疫功能重建的危险因素。结果两组基线资料差异无统计学意义(P>0.05)。治疗后,实验组CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)及淋巴细胞数目提高,差异有统计学意义(P<0.05);两组肝肾功能指标差异无统计学意义(P>0.05)。CD4^(+)>200个/μL的免疫重建成功率是<100个/μL的202.26倍(95%CI:8.49~4816.58),联合治疗成功率是HAART的7.60倍(95%CI:1.32~43.71),HAART≥5年的成功率是<5年的12.01倍(95%CI:1.73~83.29)。结论中药联合穴位贴敷有助于提升HIV/AIDS患者免疫功能,提高免疫重建成功率,具有重要的临床应用潜力和研究价值。
Objective To investigate the effect of oral administration and acupoint application of Traditional Chinese Medicine combined with highly active anti-retroviral therapy(HAART)on immune reconstitution patient with HIV/AIDS and its influencing factors.Methods A total of 90 patients with HIV/AIDS were randomly divided into the control group and intervention group,with 45 cases in each group.All patients received HAART,and those in the intervention group were given oral administration and acupoint application of Traditional Chinese Medicine for one month.Immune function indexes(CD4^(+),CD8^(+),CD4^(+)/CD8^(+)ratio,lymphocyte count)and liver/kidney function were compared at 1,3,6 months post-treatment.Logistic regression was used to identify risk factors for immune reconstitution.Results There was no significant difference in baseline data between the two groups(P>0.05).After treatment,the number of CD4^(+),CD8^(+),CD4^(+)/CD8^(+),lymphocytes in the experimental group increased significantly(P<0.05).There was no significant difference in liver and kidney function between the two groups(P>0.05).Multivariate Logistic regression analysis showed that the success rate of immune reconstitution for CD4^(+)>200 cells/μL was 202.26 times(8.49~4816.58)higher than for CD4^(+)<100 cells/μL,and the success rate of immune reconstitution in the experimental group was 7.60 times(1.32~43.71)higher than that in the control group.The success rate of immune reconstitution in patients who had received HAART≥5 years was 12.01 times(1.73~83.29)higher than those<5 years.Conclusion Based on the HAART,adjunctive oral admin⁃istration and acupoint application of Traditional Chinese Medicine may enhance immune function and improve immune reconstitution success in HIV/AIDS patients.
