摘要
目的根据“杂合以治”理论,探讨腕踝针与灸法、中药贴敷相结合,缓解多发性骨髓瘤骨病疼痛的临床效果。方法选择2025年1月—6月成都中医药大学附属医院血液科收治的68例多发性骨髓瘤骨病患者作为研究对象,随机分为对照组和干预组,各34例。对照组实施常规疼痛护理方案+穴位贴敷联合艾灸,干预组在对照组基础上增加腕踝针,两组治疗周期均为10 d。分别于治疗前、治疗后采用疼痛程度数字评估量表(NRS)和欧洲癌症研究与治疗组织生活质量测定量表(EORTC QLQ-C30)评估患者疼痛程度与生活质量,并对比两组疼痛缓解有效率。结果干预组总有效率为97.05%(33/34),高于对照组的85.29%(29/34),差异有统计学意义(P<0.05)。治疗后,两组患者NRS评分均低于治疗前,且干预组NRS评分较对照组降低更为显著,差异有统计学意义(P<0.01)。治疗后,两组EORTC QLQ-C30量表躯体功能、角色功能、情绪功能、整体健康维度评分较治疗前提升,疼痛领域评分显著降低,差异有统计学意义(P<0.01);且干预组躯体功能、情绪功能、整体健康和疼痛领域维度评分的改善幅度较对照组明显,差异有统计学意义(P<0.01)。结论腕踝针结合中医护理技术可有效缓解多发性骨髓瘤骨病患者疼痛,改善患者生活质量,提高临床疗效。
Objective Based on the theory of combination of therapeutic methods,this paper aims to investigate the clinical efficacy of combining wrist-ankle acupuncture with moxibustion and Chinese herbal paste application in relieving bone pain in patients with multiple myeloma.Methods A total of sixty-eight patients with multiple myeloma bone disease admitted to the De⁃partment of Hematology at the Hospital of Chengdu University of Traditional Chinese Medicine between January 2025 and June 2025 were selected as the study subjects.They were randomly di⁃vided into a control group(n=34)and an intervention group(n=34).The control group received a conventional pain management plan and acupoint application combined with moxibustion.The intervention group received wrist-ankle acupuncture in addition to the control group's regimen.The treatment period for both groups was 10 days.The Numeric Rating Scale(NRS)for pain in⁃tensity,the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30(EORTC QLQ-C30),and the pain relief effective rate were compared between the two groups before and after treatment.Results The total effective rate in the intervention group was 97.05%(33/34),significantly higher than 85.29%(29/34)in the control group,with a statistically significant difference(P<0.05).Post-treatment NRS scores in both groups were sig⁃nificantly lower than pre-treatment scores,and the decrease in NRS scores in the intervention group was significantly greater than that in the control group(P<0.01).The physical function⁃ing,role functioning,emotional functioning,and global health status scores in both groups were significantly higher after treatment,while pain domain scores were significantly lower,with statis⁃tically significant differences P<0.01);Increase in scores of physical functioning,emotional functioning and global health status scores,and decrease in score of pain domain,was significant⁃ly greater in the intervention group compared to the control group(P<0.01).Conclusion Wrist-ankle acupuncture combined with Traditional Chinese Medicine nursing techniques can ef⁃fectively relieve bone pain,improve quality of life,and enhance clinical efficacy in patients with multiple myeloma bone disease.
