摘要
目的探讨克罗恩病伴肛周病变患者生存质量的潜在剖面,分析不同潜在剖面的影响因素。方法采用横断面调查法,便利选取中山大学附属第六医院收治的303例克罗恩病伴肛周病变患者作为调查对象,采用自编的一般情况调查表、知觉压力量表(PSS)、炎症性肠病生存质量量表问卷(IBDQ)进行调查。以生存质量的四个维度作为外显指标进行潜在剖面分析,采用二元Logistic回归分析探讨克罗恩病伴肛周病变患者生存质量潜在类别的影响因素。结果克罗恩伴肛周病变患者的生存质量分为2个潜在剖面,包括“低生存质量-社会功能匮乏组”(96.2%)和“高生存质量-情感功能丰富组”(98.8%),2个潜在剖面在社会功能及情感功能方面上存在差异。在二元Logistic回归分析结果显示,家庭人均月收入、是否有营养风险、是否使用生物制剂、知觉压力是克罗恩病伴肛周病变患者生存质量的影响因素。结论克罗恩病伴肛周病变患者的生存质量具有差异性,提示临床工作中应重点关注社会功能差的患者,针对性制定护理干预措施,提升此类患者生存质量,为临床护理工作提供科学依据。
Objective To explore the latent profiles of quality of life(QoL)in Crohn’s dis⁃ease(CD)patients with perianal lesions and analyze influencing factors across different profiles.Methods A cross-sectional survey was conducted using convenience sampling,enrolling 303 CD patients with perianal lesions from the Sixth Affiliated Hospital of Sun Yat-sen University.Data were collected via self-designed questionnaires(demographics),the Perceived Stress Scale(PSS),and the Inflammatory Bowel Disease Questionnaire(IBDQ).Latent profile analysis(LPA)was performed using the four QoL dimensions as manifest variables,and binary logistic re⁃gression identified factors influencing QoL latent classes.Results Two distinct QoL profiles emerged:①"Low QoL-Poor Social Function Group"(96.2%)and②"High QoL-Rich Emotion⁃al Function Group"(98.8%),with significant differences in social and emotional functioning.Binary logistic regression revealed that monthly household income per capita,nutritional risk,bio⁃logic therapy use,and perceived stress were key determinants of QoL.Conclusion QoL hetero⁃geneity exists among CD patients with perianal lesions.Clinical interventions should prioritize pa⁃tients with impaired social function,tailoring nursing strategies to improve QoL.This study pro⁃vides evidence-based insights for clinical nursing practice.
