基于力学参数的血管介入操作客观评估

Objective evaluation of vascular interventional operation based on mechanical parameters

摘要 目的通过采集模拟介入过程中器械与模型的接触力,对操作者的技能进行客观评估。方法首先,构建一个介入手术操作训练平台,包括体外主动脉模型、体外仿生循环系统、视觉模块和力学模块以及实验控制终端,可采集模拟介入过程中器械与模型相互作用产生的力学数据。其次,以6名医生和15名无经验者作为不同介入水平的实验对象完成医用导管输送到主动脉硅胶模型目标血管位置的任务。选取2名医生和4名无经验者作为测试组,其余参与人员为训练组。最后,从力学数据中提取13项数据特征,通过非参数检验和相关性分析选出有效特征,利用训练组有效特征数据训练评估模型,并对测试组分别进行基于支持向量机(support vector machine,SVM)的定性分类和基于马氏距离(Mahalanobis distance,MD)的定量分析。结果在任务完成时间和力学数据上医生与新手均存在显著性差异。测试组定性分类准确率为90%,定量分析医生得分明显高于新手(67.46±12.92 vs 29.26±16.50,P<0.001),实验人员定量得分随着介入次数增加呈现上升趋势。结论提出的基于力学参数的血管介入手术操作过程客观评估方法可以提供良好的定性分类与定量评估结果,可作为未来介入医生培训的有效辅助工具。 Objective To evaluate the vascular interventional skill objectively by collecting the contact force between the instrument and the phantom during simulant invasive surgeries.Methods Firstly,a quantitative platform for interventional operation was constructed,which included a in-vitro aortic phantom,a mock circulation loop,visual and mechanical modules,as well as control terminals,capable of collecting mechanical data during the intervention process.Secondly,six experienced doctors and fifteen novices were selected as experimental subjects with different levels of intervention to complete the task of delivering medical catheters to the target vascular location of the aortic phantom.Two doctors and four novices were selected as the test group,and the rest data were selected as the training group.Finally,thirteen statistical features were extracted from the mechanical data,and the effective features were selected through nonparametric test and correlation analysis.The evaluation model was trained using the effective feature data of the training group.The test group was qualitatively classified based on support vector machine(SVM)and quantitatively analyzed based on Mahalanobis distance(MD).Results There were significant differences in task time and mechanical data between doctors and novices.In the test group,the accuracy rate of qualitative classification was 90%.Quantitative analysis showed that doctors score significantly higher than novices(67.46±12.92 vs 29.26±16.50,P<0.001),and the quantitative scores of experimenters showed an upward trend with the increase of intervention times.Conclusions The objective evaluation method based on mechanical parameters can provide good qualitative and quantitative evaluation results,and can be used as an auxiliary tool for the training of interventional doctors in the future.
