摘要
目的探讨乳腺癌术后化疗患者主要照顾者心理脱离的潜在类别特征,并分析不同类别的影响因素。方法采用便利抽样法,选取2023年12月-2024年6月南京医科大学附属苏州医院乳腺外科收治的220例乳腺癌术后化疗患者及其主要照顾者为研究对象。采用一般资料调查表、心理脱离量表、焦虑自评量表、抑郁自评量表、护理者负担量表、社会支持评定量表及家庭亲密度与适应性量表对其进行调查。通过潜在剖面分析探讨乳腺癌术后化疗患者主要照顾者心理脱离的潜在类别特征,并采用多元Logistic回归分析不同潜在类别的影响因素。结果乳腺癌术后化疗患者主要照顾者心理脱离可分为低脱离组(45.45%)、中脱离组(35.45%)、高脱离组(19.10%)3个潜在类别。多元Logistic回归分析结果显示,总照顾时长、每日照顾时间、照顾负担及社会支持是乳腺癌术后化疗患者主要照顾者心理脱离的影响因素。结论乳腺癌术后化疗患者主要照顾者心理脱离存在明显的分类特征,临床护理人员可以根据主要照顾者心理脱离不同类别特征,采取针对性的干预措施,以改善主要照顾者的心理脱离水平。
Objective To explore the categories of psychological detachment among the main caregivers of patients with breast cancer after chemotherapy and analyze its influencing factors.Methods A convenience sampling method was used to select 220 main caregivers of patients with breast cancer and who were treated in Suzhou Hospital Affiliated to Nanjing Medical University between December 2023 and June 2024 as the research subjects.They were surveyed using a general information questionnaire,the psychological detachment scale,the self-rating anxiety scale,the self-rating depression scale,Zarit caregiver burden scale,the social support rating scale,and the family adaptability and cohesion scale.Moreover,the latent profile analysis was used to explore the potential category characteristics of psychological detachment among the main caregivers,and multiple Logistic regression analysis was used to analyze the influencing factors of different potential categories.Results The psychological detachment of the main caregivers of patients with breast cancer after chemotherapy can be divided into three potential categories:low detachment group(45.45%),medium detachment group(35.45%),and high detachment group(19.10%).The results of multiple Logistic regression analysis showed that total care duration,daily care time,care burden,and social support were the influencing factors of psychological detachment among the main caregivers.Conclusion There are obvious classification characteristics of psychological detachment among the main caregivers of patients with breast cancer after chemotherapy.Clinical nursing staff can take targeted intervention accordingly to improve their level of psychological detachment.
