摘要
2023年日本重症医学会联合重症早期康复临床实践指南委员会系统制定了基于多学科协作的危重症患者早期康复治疗方案,并于《重症监护杂志》发布《日本危重症患者康复治疗临床实践指南》,系全球首部危重症患者康复治疗临床实践指南。本文对该指南进行深度解读,重点围绕成人ICU患者早期康复治疗的频率与方式、吞咽障碍的筛查与康复、下床活动和运动康复启动与停止时机、营养支持、儿童ICU患者个体化康复治疗、出院后的康复治疗及建议等临床关键问题展开系统性的分析,旨在为危重症患者的康复治疗提供标准化、可操作的临床实践参考并提出本土化建议,推动多层级医疗机构优化危重症患者从“生存救治”向“功能-社会全周期管理”的康复治疗路径。
Objective In 2023,the Japanese Critical Care Society,in collaboration with the Clinical Practice Guidelines Committee for Early Rehabilitation of Critically Ill Patients,systematically formulated an early rehabilitation treatment plan for critically ill patients based on multiple disciplinary team and published Japanese Clinical Practice Guidelines for Rehabilitation in Critically Ill Patients which was the world's first clinical practice guidelines for rehabilitation treatment of critically ill patients in the Journal of Critical Care.This article conducted an in-depth interpretation of the guideline,focusing on a systematic analysis of key clinical issues such as the frequency and methods of early rehabilitation treatment for adult ICU patients,screening and rehabilitation of dysphagia,the initiation and cessation timing of activities out of bed and exercise rehabilitation,nutritional support,individualized rehabilitation treatment for pediatric ICU patients,and rehabilitation treatment and suggestions after discharge.The aim was to provide standardized and operational clinical practice references for the rehabilitation treatment of critically ill patients and put forward localized suggestions,promoting multi-level medical institutions to optimize the rehabilitation treatment path for critically ill patients from"survival treatment"to"full-cycle management of function and society".
