摘要
目的 分析在学龄前儿童上腹部CT扫描过程中引入噪声指数结合多模型自适应迭代重建的临床价值。方法 选取2022年3月至2023年3月漯河市第二人民医院的90例儿童,所有儿童均接受上腹部CT扫描检查,根据噪声指数值差异将其分为三组(噪声指数值分别设为13、15、17),三组儿童均为30例,CT检查图像均行多模型自适应迭代重建(权重分别为0%、40%、80%,共3个亚组),并采用单因素方差分析对不同噪声指数和多模型自适应迭代重建权重条件下标准差、对比信噪比、信噪比等客观指标进行对比,同时应用Kruskal-Wallis H检验对图像主观评分进行对比分析。结果 噪声指数值持续升高时,有效辐射剂量、CT容积剂量指数、剂量长度乘积等会持续下降,噪声指数=15、17组的有效辐射剂量在1 mSv以下,图像质量系数在15以上;随着多模型自适应迭代重建权重的增大,对比信噪比等值逐渐升高,特别是权重在40%的时候图像显示效果最佳。结论 学龄前儿童实施上腹部CT检查期间选择低管电压100 kV结合噪声指数=15及多模型自适应迭代重建40%的扫描方案能有效减少辐射剂量的同时获得良好的CT图像。
【Objective】To analyze the clinical value of noise index combined with multi-model adaptive iterative reconstruction in upper abdominal CT scanning of preschool children.【Methods】A total of 90 preschool children in Luohe Second People's Hospital from March 2022 to March 2023 were selected,all of whom received upper abdominal CT scan and were divided into 3 groups(noise index values were set as 13,15 and 17 respectively)according to the difference in noise index values.The number of children in each of the 3 groups was 30.All CT images were reconstructed by multi-model adaptive iterative reconstruction(weights were 0%,40%and 80%,with a total of 3 subgroups),and one-way ANOVA was used to compare objective indicators such as standard deviation,contrast signal-to-noise ratio(SNR)and SNR under different noise index and multi-model adaptive iterative reconstruction weight conditions.At the same time,Kruskal-Wallis H test was used to compare and analyze the subjective scores of the images.【Results】When the noise index value keeps rising,the effective radiation dose,CT volumetric dose index,dose-length product,etc.will continue to decline.The effective radiation dose of the groups with noise index=15 and 17 is below 1 mSv,and the image quality coefficient is above 15.With the increase of the weights of multi-model adaptive iterative reconstruction,the contrast signal-to-noise ratio equivalent gradually rises,especially when the weight is 40%,the image display effect is the best.【Conclusion】The scanning scheme of low tube voltage 100 kV combined with noise index=15 and multi-model adaptive iterative reconstruction 40%during upper abdominal CT examination for preschool children can effectively reduce the radiation dose and obtain good CT images.
