PDCA循环管理在经皮冠状动脉介入治疗患者口服水化治疗中的应用效果

摘要 目的探讨计划-实施-检查-处置(plan-do-checkact,PDCA)循环管理在经皮冠状动脉介入治疗(percutaneous coronary intervention,PCI)患者口服水化治疗中的应用效果。方法采用便利抽样法选取2022年2—7月广西医科大学第一附属医院收治的行PCI治疗的患者466例为研究对象,根据就诊时间分组,将PDCA循环管理执行前2022年2—4月的231例患者作为对照组,PDCA循环管理执行后2022年5—7月的235例患者作为干预组,对照组给予PCI常规护理措施,干预组实施PDCA循环管理模式的护理干预。比较2组患者的口服水化达标率、术后4 h和24 h尿量及术后24 h内不良反应发生率。结果干预组患者口服水化达标率为93.19%(219/235),高于对照组的39.39%(91/231),差异有统计学意义(P<0.05)。术后4、24 h,干预组尿量分别为(861.19±73.66)mL、(2251.95±148.37)mL,多于对照组的(414.24±65.38)mL、(2005.99±113.50)mL,差异均有统计学意义(P<0.05)。术后24 h内,干预组不良反应发生率为0.85%,低于对照组的6.93%,差异有统计学意义(P<0.05)。结论PDCA循环管理应用于PCI术后患者的口服水化治疗中,可提高口服水化达标率,增加术后尿量,减少造影剂肾毒性,降低不良反应发生率,保障患者围手术期的安全性。 Objective To evaluate the effectiveness of plan-do-check-act(PDCA)cycle management in hydration therapy for patients undergoing percutaneous coronary intervention(PCI).Methods Convenience sampling was used to select 466 patients who underwent PCI treatment at the First Affiliated Hospital of Guangxi Medical University from February to July 2022 as the research subjects.According to the time of visit,231 patients from February to April 2022 before the implementation of PDCA cycle management were selected as the control group,and 235 patients from May to July 2022 after the implementation of PDCA cycle management were selected as the intervention group.The control group received routine PCI nursing measures,while the intervention group received nursing interventions based on the PDCA cycle management model.The oral hydration compliance rate,postoperative urine output at 4 and 24 hours,and incidence of adverse reactions within 24 hours between two groups of patients were compared.Results The oral hydration compliance rate of intervention group patients was 93.19%(219/235),which was higher than the control group's 39.39%(91/231),and the difference was statistically significant(P<0.05).At 4 and 24 hours after surgery,the urine output of the intervention group w as(861.19±73.66)mL and(2251.95±148.37)mL,respectively,which were higher than those of the control group(414.24±65.38)mL and(2005.99±113.50)mL,and the differences were statistically significant(P<0.05).Within 24 hours after surgery,the incidence of adverse reactions in the intervention group was 0.85%,which was lower than the 6.93%in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of PDCA cycle management in oral hydration therapy for patients after PCI can improve the oral hydration compliance rate,increase postoperative urine output,reduce contrast agent nephrotoxicity,lower the incidence of adverse reactions,and ensure the safety of patients during the perioperative period.
