摘要
目的探讨中文版简版McGill疼痛问卷-2(short-form McGill pain questionnaire-2,SF-MPQ-2)评价晚期肿瘤骨转移疼痛的信效度。方法选取2022年1月—2023年10月在福建医科大学附属第二医院就诊的268例晚期恶性肿瘤骨转移患者作为研究对象,采用SF-MPQ-2对其进行问卷调查,并验证不同人口学特征人群评分差异。采用验证性因子分析评价SF-MPQ-2的信效度。结果SF-MPQ-2量表内在一致性信度指标显示总量表Cronbach'sα系数为0.709,4个维度的Cronbach'sα系数:持续性疼痛(continuous pain,CP)为0.775,间歇性疼痛(intermittent pain,IP)为0.769,神经病理性疼痛(neuropathic pain,NP)为0.864,对情感性描述(affective descriptors,AD)为0.829。NP维度模型拟合组合信度(composite reliability,CR)最高,为0.870,而AD维度的平均提炼方差(average variance extracted,AVE)最高,为0.577。SF-MPQ-2量表折半信度Spearman-Brown系数为0.843。验证性因子结果显示,χ^(2)=734.570,自由度(degree of freedom,df)=203,χ^(2)/df=3.619,比较拟合指数、调整拟合优度指数、拟合优度指数值分别为0.789、0.768、0.814,近似误差均方根为0.099。结论中文版SF-MPQ-2在晚期肿瘤骨转移疼痛患者中具有很好的信效度,可以有效评价这类患者的疼痛程度。
Objective To explore the reliability and validity of short-form McGill pain questionnaire-2(SF-MPQ-2)in evaluating the pain of advanced cancer patients with bone metastasis.Methods A total of 268 patients with advanced cancer and bone metastasis who were treated in the Second Affiliated Hospital of Fujian Medical University from January 2022 to October 2023 were selected as the research subjects.They were surveyed using the SF-MPQ-2 questionnaire,and the differences in scores among population with different demographic characteristics were verified.Confirmatory factor analysis was used to evaluate the reliability and validity of the SF-MPQ-2.Results The internal consistency reliability index of the SF-MPQ-2 scale showed that the Cronbach'sαcoefficient of the total scale was 0.709,and the Cronbach's alpha coefficients of the four dimensions were:the continuous pain(CP)was 0.775,the intermittent pain(IP)was 0.769,the neuropathic pain(NP)was 0.864,and the affective descriptors(AD)was 0.829.The NP dimension model had the highest composite reliability(CR),with a value of 0.870,while the average variance extracted(AVE)for the AD dimension was the highest,with a value of 0.577.The Spearman-Brown coefficient for the half-split reliability of the SF-MPQ-2 scale was 0.843.The confirmatory factor results showed thatχ^(2)=734.570,degree of freedom(df)=203,χ^(2)/df=3.619,and the comparative fit index,adjusted goodness of fit index,and goodness of fit index values were 0.789,0.768,and 0.814,respectively.The root mean square error of approximation was 0.099.Conclusion The Chinese version of the SF-MPQ-2 has good reliability and validity in evaluating the pain of patients with advanced cancer and bone metastasis,andcaneffectively evaluate the pain level of such patients.
