摘要
目的 探讨舒肝解郁胶囊对失眠伴抑郁患者cAMP/CREB/BDNF信号通路的影响。方法 选取2024年6月—2025年5月在三门峡市康复医院诊治的50例失眠伴抑郁患者,随机数字表法分为对照组和观察组各25例,统计两组疗效及不良反应,记录两组治疗前、治疗后中医证候积分、cAMP/CREB/BDNF信号通路及神经递质水平。结果 治疗后,观察组总有效率高于对照组(P<0.05)。治疗前,两组情绪低落、胸胁胀满、食少纳呆、急躁易怒、疲乏无力、善太息、胸闷、紧张不安、多梦、腹痛评分比较(P>0.05);治疗后与治疗前相比,两组情绪低落、胸胁胀满、食少纳呆、急躁易怒、疲乏无力、善太息、胸闷、紧张不安、多梦、腹痛评分均降低;与对照组相比,观察组情绪低落、胸胁胀满、食少纳呆、急躁易怒、疲乏无力、善太息、胸闷、紧张不安、多梦、腹痛评分更低(P<0.05)。治疗前,两组cAMP、CREB、BDNF水平比较(P>0.05);治疗后与治疗前相比,两组cAMP、CREB、BDNF水平均上升;与对照组相比,观察组cAMP、CREB、BDNF水平均明显上升(P<0.05)。治疗前,两组γ-氨基丁酸、5-羟色胺水平比较(P>0.05);治疗后与治疗前相比,两组γ-氨基丁酸、5-羟色胺水平均上升;与对照组相比,观察组γ-氨基丁酸、5-羟色胺水平均升高(P<0.05)。治疗过程中,对照组出现1例轻微乏力,2例头晕、1例眼上翻、1例运动迟缓,不良反应发生率为20.00%(5/25),观察组出现1例轻微乏力,不良反应发生率为4.00%(1/25),观察组不良反应发生率低于对照组(P<0.05)。结论 舒肝解郁胶囊能够通过调控cAMP/CREB/BDNF信号通路发挥治疗失眠伴抑郁患者的作用。
Objective To explore the effect of Shugan Jieyu Capsule on the cAMP/CREB/BDNF signaling pathway in patients with insomnia and depression.Methods Fifty patients with insomnia and depression who were diagnosed and treated in our hospital from June 2024 to May 2025 were selected and randomLy divided into a control group and an observation group,with 25 cases in each group.The therapeutic effects and adverse reactions of the two groups were statistically analyzed.The TCM syndrome scores,cAMP/CREB/BDNF signaling pathway and neurotransmitter levels before and after treatment were recorded in both groups.Result After the treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).Before treatment,the scores of low mood,fullness and distension in the chest and hypochondrium,poor appetite,irritability,fatigue,excessive breathing,chest tightness,nervousness,frequent dreams and abdominal pain were compared between the two groups(P>0.05),after the treatment,compared with before the treatment,the scores of low mood,fullness and distension in the chest and hypochondrium,poor appetite,irritability,fatigue,excessive breathing,chest tightness,nervousness,frequent dreams and abdominal pain in both groups decreased.Compared with the control group,the observation group had lower scores for low mood,fullness in the chest and hypochondrium,poor appetite,irritability,fatigue,excessive breathing,chest tightness,nervousness,frequent dreams and abdominal pain(P<0.05).Before treatment,the levels of cAMP,CREB and BDNF in the two groups were compared(P>0.05);After treatment,compared with before treatment,the levels of cAMP,CREB and BDNF in both groups increased.Compared with the control group,the levels of cAMP,CREB and BDNF in the observation group increased significantly(P<0.05).Before treatment,the levels of gamma-aminobutyric acid and serotonin in the two groups were compared(P>0.05);After treatment,compared with before treatment,the levels of gamma-aminobutyric acid and serotonin in both groups increased.Compared with the control group,the levels of gamma-aminobutyric acid and serotonin in the observation group were both increased(P<0.05).During the treatment process,in the control group,there was 1 case of mild fatigue,2 cases of dizziness,1 case of up ward eye roll,and 1 case of bradycardia.The incidence of adverse reactions was 20.00%(5/25).In the observation group,there was 1 case of mild fatigue,and the incidence of adverse reactions was 4.00%(1/25).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion Shugan Jieyu Capsule can exert its therapeutic effect on patients with insomnia and depression by regulating the cAMP/CREB/BDNF signaling pathway.
